I, JHOANNA LOU G. OBLIGADO, am the legal parent/guardian of LORAINNE STACEY G. OBLIGADO,

a student currently enrolled at Tarlac State University – Laboratory School, hereby grant my consent for my child
to participate in the “Crescence: Awaken the Harmony,” an event organized outside the university premises.

I am aware that the university is not directly in charge of overseeing or organizing this event, and I hereby release
the university, its staff, and its representatives from any liability regarding my child's involvement in the
competition. Inherent risks are associated with any activity and by granting permission for my son/daughter to
participate, I acknowledge that such risks exist. However, I believe that the learning opportunity outweighs these
risks and I hereby grant permission for my son/daughter to participate.
