UNDERTAKING OF GOOD CONDUCT AND WAIVER DURING THE ON-

THE-JOB TRAINING

I, ______________________________________________________________, _______ years old, __________, a student of Cavite

University Main Campus, having been selected as one of the On-the-Job trainees at
_____________________________________ hereby undertake the following good conduct and waiver
during the entire training period:
1.During the entire period, I promise to observe all rules of ethics and codes of behavior at all times.
2.I will treat the supervisors and superiors of the linkage establishment with outmost respect and
courtesy at all times.
3.I will give the guest and customers of the linkage establishment the appropriate service according to
the job assigned to me.
4.I will maintain friendly relationships at all time with my peers and the linkage establishment
employees.
5.I will maintain professionalism in all my activities as required by the linkage establishment.
6.I will observe the rules and regulations of the linkage establishment with regards to:
a. My working/training hours;
b. My dress code during my working/training hours; and
c. My duties and responsibilities as assigned by the immediate supervisor or superior.
7.I wil not be absent from my duty during my training period.
8.I will perform the on-the-job training coordinator of any concern pertaining to the training,
attendance, tardiness, behavioral and attitudinal problems.
9.In case of my absence from my training duties due to physical sickness, I will present a medical
certificate.
10. I will understand that my unauthorized absence during my training period will be subjected to
disciplinary action by both the linkage establishment and Cavite State University Main Campus
according to the respective policies with regards to absences.
11. I will accept and accomplish all the duties and responsibilities given to me as part of the training.
12. I will stay away from any personal problems and intrigues of the employees in the linkage
establishment.
13. I will not interrupt in any manner the official on-the-job training schedule under which I am
placed.
14. I will avoid smoking and drinking alcoholic beverages. I will not consume prohibited drugs within
the premises of the linkage establishment.
15. I will not loiter within the premises of the linkage establishment during my duty hours. I will not
stay within the linkage establishment premises for more than thirty (30) minutes after my
work/training for the day.
16. I will not commit any offense which would result in disciplinary action by the linkage
establishment or Cavite State University Main Campus, such as:
 a. Acts of dishonesty;
b. Acts of immorality;
c. Commission of a criminal offense;
d. Acts of destroying the goodwill of the linkage establishment and/or Cavite State
University Main Campus;
e. Acts of disturbance of peace and order in the linkage establishment;
f. Acts of violence against the staff, superiors and co-employees of the linkage
establishment;
g. Creating loss, damage or disappearance of linkage establishment properties;
h. Acts of violation of policies, rules and regulations in the linkage establishment; and
i. Acts of disorderliness.
17. I understand that any violation of my above undertaking or other forms of misdemeanor shall be
subjected to disciplinary action. After thorough investigation by the representatives of the linkage
establishment and Cavite State University Main Campus, I may be subjected to any of the
following disciplinary actions depending on the gravity of the misdemeanor:
a. Written reprimand;
b. Suspension from the training for a period of time as deemed necessary by the linkage
establishment or Cavite State University Main Campus; and
c. Termination from the training.
18. I understand that if the number of hours required from the training fall short of the required
number of hours as agreed upon due to any disciplinary action, I will have to undergo the OJT
again.
19. I understand that this On-the-Job Training does not establish employee’s relationship between me
and the linkage establishment.
20. In any injury, illness, damage or accident as a result of my negligence that may occur during the
period of the on-the-job training, I will waive financial obligations from the linkage establishment
and Cavite State University Main Campus.

This waiver takes effect only on the duration of the said on-the-job training.
