MPS, for the 2nd Quarter of S.Y. 2017-2018

1. In pursuant to Division Memorandum No. 270, s. 2017, this office hereby directs all school heads
of this district to submit the TOS, Test Items, Tally of Correct Responses and MPS to this office
on or before November 13, 2016. (Attention: Harry V. Letrada-Elem,; Leonora L. Venezuela-SH)

2. For prompt computation and consolidation, all schools are directed to use the electronic MPS and
TCR program developed by Fhebe S. Selorio presented during INSET.

3. All documents related to this report should pass the district office for proper verifications before
forwarding it to the division office.

4. Proper labelling and sequencing of files should be observed in the packaging of the folders to
ensure uniformity of MPS reports in our district.
