At your request and upon our site evaluation, Cappa Inc. is pleased to submit this proposal for the

abatement of ACM Floor Tiles in the Basement at the above-mentioned premises.

SCOPE OF WORK:

The scope of work shall be the removal of ACM Floor Tiles on the 1st Floor:

1. Remove approx. 361sf of ACM Floor Tiles in the Living Room.

2. Remove approx. 154sf of ACM Floor Tiles in the Kitchen.
3. Remove approx. 128sf of ACM Floor Tiles in Hallway.
4. Remove approx. 20sf of ACM Floor Tiles in the Bathroom.

Total Square Footage 697 SF.

GENERAL TERMS:

1. Cappa Inc. shall provide all licensed labor, materials, tools, and Supervision necessary to remove
and disposal of the asbestos containing materials as referenced above in a timely and
workmanlike manner.
2. The Homeowner/GC shall be responsible for the provision of water, electrical power, security,
and access to the site for the duration of work.
3. The Homeowner/GC shall be responsible for hiring an Independent Third-Party Air Monitoring
Company for the duration of this project as required by the NYS-DOL Asbestos Rules and
Regulations.
4. The cost for the Independent Third-Party Air Monitor IS NOT included in the price quoted
above.
5. The Homeowner/GC will ensure that all work areas are cleared to allow workers access to
work areas.
6. Any additional work or deviation from the scope of the work herein shall only commence upon
execution of a written change order by both parties.
