A. DOCUMENTATION RELATING TO THE PURPOSE OF THE JOURNEY
Registration certificate (C.C.I.A.A.) in the register of enterprises and, in case of a foreign employer, with an analogous attestation certified by the competent Italian diplomatic-consular Representative (in case of a contract with the customer abroad) Availability of a suitable accommodation Copy of the "Dichiarazione di Responsabilità" (Declaration of Responsibility) previously issued or sent by the customer (or by his legal representative) to the competent "Direzione Provinciale del Lavoro, Servizio Ispezioni Provinciale del Lavoro", which indicates that by virtue of the stipulated contract, no contract of "lavoro subordinato" (subordinate work) will be established "Nulla Osta" (authorization) issued by the Questura (local Police) of the town where the activity will take place A work contract, with which independent workers are assured of a compensation superior to the minimum level provided by the law for exemption of the participation of health expenses (Euro 8.400) Copy of the income last balance sheet deposited with the register of enterprise (capital firm), or of the last declarations of revenue (firms of persons or individual enterprise)

APPLICANTS SHOULD NOTE THE FOLLOWING:
The Consulate General of Italy reserves the right to demand for any additional document if deemed essential. All the documents should be provided in original and copy. Original documents (except bank statements) will be returned to applicants while copies will be kept with the visa application. Any forged/fake document or passport submitted will not be returned to the applicant who will be prosecuted in the Italian and the Nigerian Court.
