As already intimated to you that My PSA level is on higher side and as per advice of the doctor biopsy
has to done for the prostate nodules which are indicated in MRI scan.My appointment for biopsy is fixed
for today 19-3-24 (Afternoon) at Randhawa X-Ray and Ultrasound Center , Mall Road , Amritsar.As
advice of the doctor, Kindly grant me causal leave for further three days i.e. 20-3-24 to 22-3-24 and
station leave till 9 am 26-3-24.
Photocopy of Prescription of doctor and other reports attached for your reference please.
