JOINT AFFIDAVIT FOR DELAYED REGISTRATION OF BIRTH

We, PABLITO SILVA and ANTONIA CABISCUELAS both of legal ages, Filipino citizens and
with respective residence at Brgy. Bilucao, Malvar, Batangas after having been duly sworn to in
accordance with law do hereby depose and state that:

That we can personally attest to the fact that FELIMON CABISCUELAS SILVA who was
born November 22, 1969 at Bilucao Malvar, Batangas to Antonia Cabiscuelas and
Pablito Silva;

That through inadvertence and oversight, the Certificate of Birth of FELIMON

CABISCUELAS SILVA was not immediately facilitated and recorded with the Office of the
Municipal Registar of Malvar;

That we can personally attest to the veracity of the foregoing statements as we have
been their former neighbors and who have lived in the same community for quite a long
period of time; and

That we are executing this Joint-Affidavit for Delayed Registration of Birth to attest to
the truth of all the foregoing statements and in support of the delayed registration as to
the fact of birth FELIMON CABISCUELAS SILVA.

IN TRUTH WHEREOF, we have here unto set our hands on this 2nd day of July, 2024 at
Malvar, Batangas.
