AFFIDAVIT OF DISCREPANCY

I, ISABELO O. LAT, Filipino, married, of legal age, resident of Barangay San Pedro II
Malvar, Batangas, Philippines, after having been sworn by law hereby depose and state
that:

That in my Certificate of Live Birth as recorded in the Office of the Civil Registrar of
Malvar, Batangas, Philippines, and Philippines Statistic Authority, it is stated that I was
born 08 July 1965 in Malvar Batangas;

That my relatives, acting on the honest belief that I was born on 08 July 1965, had used 08
July 1965 as my birth date in my Birth certificate and also in all my other government
issued ID ;

That When I filled up a form in applying for a Driver’s License required by the Land
Transportation Office, I had inadvertently and erroneously written the date of my birth as
16 July 1965 instead of 08 July 1965;

I am executing this affidavit of Discrepancy to attest to the truth of the foregoing facts and
to explain the discrepancy in my certificates of live birth, National ID, and Driver’s License
as regards my date of birth;

Further, I am declaring that from this date hereon I will be using my true and correct date of
birth of 08 July 1965 in all my papers, records, and other documents.

IN WITNESS WHEREOF, I have hereunto set my hand this 20th of June, 2024, in Malvar,
