The program aims to aid learners’ gaps and issues regarding the least learned
skills for the quarter enabling them to be ready for the following skills for the next
quarter.

V. Monitoring and Evaluation (How shall the program be monitored and


evaluated? Who will be monitoring the program implementation? What are
their roles and functions? How will they monitor? How often will the
monitoring be? What tools will they use?)
The program will be evaluated by the school principal and academic coordinator.
They are going to check the modules to be used in the program. They are going to
monitor the performance of the students who need remediation through remediation
classes (or parents, home visitation for additional data). They will evaluate the
performance through the learning intervention program report.
