URETHRAL DIALATATION

Anaesthesia: local anaesthesia used by lignocaine jelly

Position: patient lies supine on bed

Technique: it is always wise to select a medium size bougie first.

The bougie is well lubricated with jelly and taken in right hand. With the left hand the penis is
held vertically upwards to make the urethra “j” shaped to adapt the curve of bougie. The tip of
instrument is pushed through the external meatus when the long arm of instrument will be lying
parallel to the inguinal ligament of opposite side. The instrument is slowly pushed into urethra
and the right hand will be automatically come to the midline of abdomen and long arm of
instrument will be lying along midline of the abdomen. At this stage, the resistance of perineal
membrane is felt. By this time, more or less the whole length of urethra has been traversed by
bougie and long arm of bougie will be lying vertically upwards. Now a slight depression on the
handle of bougie will push the tip of instrument into bladder. In fact, the bougie should pass to
urinary bladder by its own weight. The instrument into bladder is noted by

1) The instrument is exactly on midline of body.

2) The handle of bougie can be rotated freely as the tip of instrument is lies within the cavity
of bladder.

3) There is no bleeding.
