PUTRAJAYA – KLIA HIGHWAY PROJECT (MEX 2)

1.0 CHRONOLOGY OF THE EVENT

The incident took place at Bridge 12 deck slab area. At that time, all workers form H&S Advance
Construction PLT & staff already at site to perform their daily activity. There are a few operation team
from Bridgex (BBSB) and Maju Holding (MHSB) doing routine inspection on site.

Suddenly En. Mohd Saufi, their site supervisor smelled burnt and spotted fire near to their bending
machine located at deck slab. Due to the emergency, an evacuation procedures were triggered. The
chronologies of the event as per below:

1.0 CHRONOLOGY OF THE EVENT

1 Fire triggered.
En. Mohd Saufi spotted the fire and shouted ‘fire’ 3 times to alert everyone at the
location.
11:04 am
2 En. Nik Hazri (Bridgex Site Supervisor) who also at the incident scene immediately
instructed all workers and staff to stop work and abandon the work area and
gathered at the assembly point.

En. Nik Hazri contacted En. Muhammad Nizam (Bridgex Site Safety Supervisor) to
reported about the incident.
3 11:05 am At the same time En. Mohd Saufi together with En. Farouk Omar (Kepala)
immediately extinguish fire using ABC type fire extinguisher which located nearby
the bar cutting machine.

4 11:07 am Fire completely extinguished.

5 11:10 am All staff and site workers reach assembly area.

En. Nik Hazri start head count to ensure all personnel safely evacuated. He also

asked all personnel to report to him if there is any injuries during evacuation.

En. Nik Hazri reported to En. Muhammad Nizam and inform all workers and staffs
7 11:13 am
are safely evacuated from the work area and evacuation completed.

En. Muhammad Nizam inform MHSB Safety and announced the Fire Evacuation
8 11:14 am
Drill is ended.

Emergency evacuation procedure briefed to all staff by BBSB HSE Officer , En.

Mohd Idham followed by fire safety briefing.

9 11:30 am Firefighting training and demonstration. All personnel is participated.

10 11.55 am Firefighting training and demonstration ended and group photo session with all staff.

2.0 FIRE DRILL REPORT& EVALUATION

• All workers evacuate from workplace using both main access without panic.
