JSA DEFINITION
Job Safety Analysis:
 Is a structured approach which analyses jobs, breaks
it down into discrete steps, analyses the hazards
associated with those steps, documents precautions
and assigns responsibilities for their implementation
with the aim of ensuring a safe job execution.
 It focuses on the relationship between the worker, the
task, the tools, and the work environment.
 After identification of uncontrolled hazards, steps to
eliminate or reduce them to an acceptable risk level to
be taken.

IMPORTANT DEFINITIONS
High Risk Activity:
The activity which involves the significant hazards which could lead to the immediate danger to health, safety,
environment and property. Such activities include but not limited to:
 Confined Space Entry
 Critical Lifts
 Open Flame Hot Work
 Pyrophoric Material Handling
 Work At Heights (which required detailed rescue plan)
 Radiography
 Line Break (in which line break permit required)
 Hydro Blasting (greater than 40,000 psig)
 Hot Tap
 Online Seal Clamping
 The jobs that requires variance to any safety procedure.

IMPORTANT DEFINITIONS
 Job Performer Supervisor: Q-Chem employee who; review and approve the
JSA process, has received JSA training per the Global Training Matrix, and
has knowledge of the hazards of the job being analysed. The first level of
supervision of JSA facilitator. Normally maintenance engineer within the
maintenance organisation and engineers within the project management.
 Area Owner Supervisor: Q-Chem employee, who has received JSA training
per the Global Training Matrix. Area Owner of the area where job shall
perform and approves the JSA.
 JSA Facilitator: Q-Chem employee/baseload contractor who; leads the JSA
process, has received JSA training per the Global Training Matrix, prepare
the JSA and have knowledge of the task that performed. Normally job
performer representative acts as a JSA Facilitator.
 Major Turnaround (TA): An event planned one to two years in advance and
occurring every 4 or 5 years wherein an entire process unit of a plant is
sufficiently deinventoried and taken offline for an extended period of time
to perform maintenance and to inspect, test, add, and replace process
materials and equipment as needed.

ROLES AND RESPONSIBILITIES
 Job Performer Supervisor : Nominate Facilitator for
JSA. Sign on the JSA worksheet (Attachment HSE-SAF-
SFM-00-0018) and then send to area owner
supervisor for his signature and approval.
 Area Owner Supervisor : Approves JSA before use.
Shall sign on the JSA worksheet.
 JSA Facilitator: Shall discuss all the job steps, identified
hazards and mitigations with the lead worker as
identified in the JSA. Ensure approved and relevant
JSA shall be available at site all times. Shall sign on the
JSA worksheet as an facilitator.
 JSA Team: Help identify hazards and control measures
to reduce the risk to As Low As are Reasonably
Practicable (ALARP). Assist in the identification of any
improvements and deficiencies in the work process.

RELATIONSHIP TO PERMIT TO
WORK SYSTEM
 The results of a JSA for a work
job shall be entered in Section B
of the Work Permit or, if too
extensive, then at least the main
hazards and precautions shall be
entered into section B of the
Work Permit.
 JSA required for any task
performed under a Permit to
Work (PTW) shall be cross
referenced onto the PTW
Section B and JSA Worksheets
shall be attached to the PTW.

ADDITIONAL INFORMATION
JSA Validity:
 JSA shall be valid for a maximum of 3 months from the date it was prepared,
provided no change in job scope or job condition.
Revalidation:
 If the same job continues for more than 3 months or have to perform again after
3 months, the JSA shall be revalidated by doing the JSA process again with the
original JSA team (or its suitable substitute).
 Record Keeping:
 All soft copy of the approved, signed, and updated JSA must be keep for future
reference.
Note:
 All are encourage to create JSA using online application
 Approved, signed, and updated JSA must be attached before closing

WHEN TO CONDUCT A JSA
Area owner supervisor shall determine the need for JSA on a job. The JSA shall consider the
following when determining the need for a JSA:
 Jobs with the past injury or illness history and Jobs with the potential to cause severe or
disabling injuries or illness, even if there is no history of previous accidents.
 Previously prepared JSA for the same or similar work.
 Working on or with electrical equipment (e.g. switchgear, substations, motor control
centres, MCC buses and similar power distribution equipment) where there is a potential
for contacting electrically energized components.
 Jobs done for the first time that could present new safety challenges and jobs that are
new to operation or have undergone changes in processes and procedures.
 Job that involve multiple work crews.

WHEN TO CONDUCT A JSA
 Jobs having potential of exposures to hazardous gases at levels above their
Occupational Exposure limits (OEL). (e.g. H2S > 10 PPM).
 Working with hazardous chemicals where direct contact is possible e.g.
caustics acids, steam, chlorine, etc. unless covered by an approved work
procedure
 Jobs having higher risk (refer definition of High Risk Activities) or unusual
situations. Shall include unusual conditions such as weather extremes,
work hours, job locations, etc.
 Non-routine energy isolation or changeover of high voltage breaker which
could have process impact (e.g. 132 KV breaker switchovers).

JSA PROCESS FLOW

JSA PROCESS (JSA Team and Field
Tour)
 The team shall consist of at least three members (e.g: JSA facilitator, contractor or the person
who perform the job and or area owner supervisor designee etc.) having knowledge of the
hazards in the job being analysed. Specialists such as Safety Professionals or Training Coaches
may be included to provide additional resources to the team.
 All high risk activities shall involve HSE Department representative (Safety Specialist, Safety &
Emergency Response Specialist, Process Safety & Compliance Coordinator, Industrial Hygienist
or FSC) in JSA process.
 It is mandatory to confirm and fulfil pre checks in JSA Worksheet (Attachment HSE-SAF-SFM-00-
0018) prior to proceeding further steps of JSA.
 The JSA team shall conduct a field tour of the facility being studied to look into location specific
conditions such as access and egress, escape routes, high noise, surrounding equipment and
structure etc.

MANDATORY REQUIREMENT

It is mandatory to confirm and fulfil JSA General Information and JSA Pre Checks in JSA
Worksheet (HSE-SAF-SFM-00-0018) prior to proceeding further steps of JSA.

BREAKING JOB IN STEPS
 Steps are in sequence (first task to last)
 Job steps are not to be too broad or too
detailed
 It is important not to break the steps down into
too many items.
 The job steps that are common and which does
not require the JSA to be prepared if perform
individually, it shall be recommended not
include in the JSA. For example UNIT ENTRY etc.
 Each job step must start with ACTION words
such as: “remove” “open” “lift “etc.

IDENTIFYING THE HAZARDS
Ask the questions below when identifying the hazard for each job
steps:
• “What could go wrong?”
• “Who might be harmed?”
Hazards categories:
• Environmental – Weather Condition
• Electrical – Live wire, multiple extension
• Physical – Sharp edge, Falling hazard, Slip and trips
• Chemical – Caustic or Acid handling
• Biological – Any virus comes from living things
• Ergonomics – Awkward position, over stretching
• Psychological, etc – Work pressure, personal problem

Note: JSA Hazards Checklist shall be filled and attached with


each JSA

HAZARDS CONTROL MEASURES
The final step of the job safety analysis is to develop effective control measures to
ensure job is carried out safely:
 The JSA team shall examine each of the job steps and associated hazards and
provide solutions to control the hazards in order to minimise the risk to as
low as reasonably practical (ALARP) level. These are:
 Find an entirely new different way to do the job.
 Change the physical condition that creates the hazard.
 Revise/Change the procedure.
 Limit the use of job by reducing frequency with which it is done
(particularly helpful in maintenance &, material handling).
 If any hazards exist that pose an immediate danger to an employee’s or
contractor’s life or health, take immediate action to protect the worker.
 Each control measure or precaution shall be specific and general statements
such as “work carefully” shall be avoided.
 The control measures shall be doable and work crew shall know and
understand and shall be able to do it.

BEFORE THE JOB BEGINS
 Job must not start unless the controls and precautions specified in JSA are in
place.
 For extended or multiple shifts, Area Owner Supervisor shall verify that
approved and relevant JSA is attached.
 For non-high risk activities, JSA to be approved once by Area Owner
Supervisor and not required approval in every shift.
 For high risk activities area owner supervisor shall review and sign the JSA in
every shift.
 It is particularly important to review job safety analysis if an illness or injury
occurs on a specific job
 Contents of the JSA must be understood by all personnel involved in the job.
 For filling out of Permits prior to job start
 To describe hazards and controls during Tool box meeting

Job must be STOPED, inform your

12. PERMIT ISSUER
Permit issuer shall discuss the approved JSA with lead worker before issuing the permit

Lead Worker reviews the JSA with work crew in toolbox talk after receiving the permit

Required to understand every step of JSA before starting the job and follow the
requirements of JSA and PTW while performing the job.

SUMMARY / CONCLUSION
Things to Remember:
1. Identify the Job Scope and Form a Team
2. Confirm and Fulfil the JSA General Information and Pre Checklist which
Include tour visit and completing the hazard identification in the Field
3. Identify Job Steps, Hazard (using the completed Hazard Checklist), and
Mitigation
4. Facilitator and Job Performer Supervisor sign on the JSA worksheet and then
send to area owner supervisor for his signature
5. Area Owner Supervisor approves JSA before use and sign on the JSA
worksheet
6. Prior of permit issuance, Permit Issuer discuss to the Lead Worker the content
of the JSA ensuring that all control measures are implemented before
commencement of job and also to ensure all conditions and requirements are
fulfilled.
7. Lead Worker will conduct Toolbox Talk with all the work crew discussing the
JSA content (Job steps, Hazards Identify, and the Mitigation). and ensure work
crew understand the conditions and requirements specified in the JSA
Worksheet.
