AFFIDAVIT OF UNDERTAKING

I, _Andrew A. Lim Jr._of legal age, Filipino and currently

residing at _Lunas Street Pagatpat Cagayan de Oro City_ after having
been duly sworn in accordance with law, hereby depose and say:

1. I applied for the position of ___Meat Cutter / Butcher___

which is covered by a job order from the United Kingdom
which job order is duly accredited at FMW HUMAN
RESOURCES INTERNATIONAL CORPORATION, a licensed
recruitment agency.

2. FMW HUMAN RESOURCES INTERNATIONAL CORP. gave

me a three-week time table for the approval of my
application by the selection committee of the foreign
employer. Likewise, I was informed that there is no
guarantee that my application will be approved or I will be
selected for the said position.

3. Given the fact that I have to wait for three weeks to know
the outcome of my application at FMW HUMAN
RESOURCES INTERNATIONAL CORPORATION, I hereby
undertake to not to apply to other recruitment agencies
during the said three-week period

4. That after waiting for three weeks and there is still no

word about my application this undertaking is
automatically revoke which means that I could freely
apply to other recruitment agencies.

5. That the three-week period starts upon signing of

application letter and the on the date of execution of this
affidavit.

 6. I voluntarily sign this undertaking fully aware of its
consequences.

7. I am executing this affidavit knowingly, willingly and

voluntarily to attest to the truth of the foregoing and for
whatever legal purpose this may serve.

8. Ang salaysay po na eto ay binasa at ipinaliwanang sa wika

na aking naiitindihan at ako ay hindi pinilit , tinakot at
biinayaran para gawin eto.
