Subject: NOTICE OF DISHONOR AND FINAL DEMAND FOR PAYMENT

This letter is written with reference to the following checks, which you drew
and issued in favor of ___________________________, to wit:

Please be informed that the above-mentioned checks were dishonored by the

drawee bank upon due presentment for having been DRAWN UPON
INSUFFICIENT FUNDS (DAIF) and ACCOUNT CLOSED (ACC).

Accordingly, I am writing you to give you NOTICE OF DISHONOR of the above-

stated checks and to make a FINAL DEMAND for payment of the full value of
the checks within five (5) days from receipt of this letter.

Otherwise, much to our regret, I will be constrained to file the proper civil
and/or criminal case/s against you that may be found warranted under the
facts and circumstances.

Please know that as much as possible my client does not want to resort to any
court action to enforce her rights under the afore-mentioned checks and as
such she hopes that this matter will be solve amicably outside of court.
