Praise and gratitude we pray to the presence of Allah S.W.T, for all the abilities of His
grace and guidance so that the writer can complete the assignment of a paper entitled
"Hospital Organizational Structure" in English courses. By giving thanks to the presence of
Allah SWT for all His blessings and gifts, and not forgetting sholawat and greetings to the
great Prophet Muhammad Swt. for His guidance and treatise, which has brought the dark
ages to the bright era, and for the blessings and encouragement of various parties who have
helped us provide references in the preparation of this paper. Especially to Google search
which played a big role in making this paper. We are aware that there are still many
shortcomings in the preparation of this paper, therefore we really appreciate suggestions and

INTRODUCTION
A. BACKGROUND
Health is a healthy state, both physically, mentally, spiritually and socially which enables
everyone to live productively socially and economically. Resources in the health sector are all
forms of funds, personnel, medical supplies, pharmaceutical preparations and medical
devices as well as health service facilities and technology used to carry out health efforts
carried out by the government, regional governments, and/or the community (Depkes RI,
2009) . The hospital, which is one of the health facilities, is a reference for health services
with the main function of organizing health efforts that are healing and recovery for patients
(Ministry of Health RI, 2004). The role of the Hospital is urgent, therefore getting to know
the Hospital is not only seen from the number of patients, but rather the benefits and roles and
functions. The following will discuss the Paper on Classification and Organizational
Structure of Hospitals.
Kesehatan adalah keadaan sehat, baik secara fisik, mental, spritual maupun sosial yang
memungkinkan setiap orang untuk hidup produktif secara sosial dan ekonomis. Sumber daya
di bidang kesehatan adalah segala bentuk dana, tenaga, perbekalan kesehatan, sediaan farmasi
dan alat kesehatan serta fasilitas pelayanan kesehatan dan teknologi yang dimanfaatkan untuk
menyelenggarakan upaya kesehatan yang dilakukan oleh pemerintah, pemerintah daerah,
dan/atau masyarakat (Depkes RI, 2009). Rumah sakit yang merupakan salah satu dari sarana
kesehatan, merupakan rujukan pelayanan kesehatan dengan fungsi utama menyelenggarakan
upaya kesehatan yang bersifat penyembuhan dan pemulihan bagi pasien (Depkes RI, 2004).
Peran Rumah Sakit merupakan hal yang urgen, maka dari itu mengenal Rumah Sakit tidak
saja dilihat dari banyaknya pasien, namun lebih kepada manfaat dan peran serta fungsinya.
berikut akan dibahas mengenai Makalah tentang Klasifikasi dan Struktur Organisasi Rumah
Sakit.

CHAPTER II

THEORETICAL STUDY OR DISCUSSION

A. HOSPITAL STRUCTURE ORGANIZATION


Based on RI Law number 44 of 2009 article 33 concerning hospitals, every hospital must
have an effective, efficient and accountable organization. The hospital organization consists
of at least the head of the hospital or the director of the hospital, elements of medical
services, nursing elements, medical support elements, medical committees, internal
inspection units, as well as general and financial administration.
If concluded, the Hospital Organizational Structure consists of:
1. Head of Hospital or Director
The head of the hospital or the director of the hospital as referred to is the highest
leader with the name of the position of head, main director or director. The head of the
hospital or the director of the hospital leads the administration of the hospital.
In carrying out their duties, the head of the Hospital or the director of the Hospital
carries out the following functions:
a. coordinating the implementation of tasks and functions of organizational
elements;
b. stipulation of hospital administration policies in accordance with their
authority;
c. implementation of duties and functions of the Hospital;
d. fostering, supervising, and controlling the implementation of duties and
functions of organizational elements; And
e. evaluation, recording, and reporting.
2. Service elements in the medical field
The medical service element is an organizational element in the field of medical
services which is under and responsible to the head of the hospital or the director of the
hospital. The medical service element is led by a director, deputy director, head of field, or
manager. The medical service element is in charge of carrying out medical services.
In carrying out its duties, elements of medical services carry out functions:
a. preparing a plan for providing medical services;
b. coordination and implementation of medical services;
c. implementation of quality control, cost control, and patient safety in the field
of medical services; And
d. monitoring and evaluation of medical services.
Elements of medical services include outpatient, inpatient, and emergency services.
3. Elements of Nursing
Nursing is an organizational element in the field of nursing services that is under and
responsible to the head of the hospital or the director of the hospital. Nursing is led by a
director, deputy director, head of department, or manager. responsible for providing nursing
services.
In carrying out its duties, the elements of nursing carry out functions:
a. preparing a plan for providing nursing services;
b. coordination and implementation of nursing services;
c. implementation of quality control, cost control, and patient safety in the field
of nursing; And
d. monitoring and evaluation of nursing services.
4. Medical Support Elements
Medical support elements are organizational elements in the field of medical support
services that are under and responsible to the head of the hospital or the director of the
hospital. Medical support elements are led by directors, deputy directors, heads of
departments, or managers. Medical support elements are tasked with carrying out medical
support services.
In carrying out the duties of medical support elements carry out functions:
a. preparing a plan for providing medical support services;
b. coordination and implementation of medical support services;
c. implementation of quality control, cost control, and patient safety in the field
of medical support services;
d. management of medical records; and e. monitoring and evaluation of medical
support services.
Hospitals can form elements of non-medical support services according to needs. The
head of the hospital or the director of the hospital determines the scope of services or fields
that are included in the elements of medical support services and elements of non-medical
support services.
5. Medical Committee
The Medical Committee is an organizational element that has the responsibility to
implement good clinical governance. The Medical Committee is formed by and is responsible
to the head of the Hospital or the Hospital director.
The Medical Committee is tasked with increasing the professionalism of medical staff
working in hospitals by:
a. conduct credentials for all medical staff who will perform medical services at
the hospital;
b. maintaining the professional quality of medical staff; And
c. maintaining discipline, ethics, and professional behavior of medical staff.
In carrying out credential duties, the Medical Committee carries out functions:
a. preparation and compilation of a list of clinical authorities according to input
from a group of medical staff based on applicable professional norms;
b. carrying out examinations and assessments of competency, physical and
mental health, behavior, and professional ethics;
c. evaluation of continuing medical or dental professional education data;
d. interview the applicant for clinical authority;
e. adequate clinical authority assessment and termination;
f. reporting the results of the credential assessment and submitting clinical
authority recommendations to the medical committee;
g. carrying out the recredential process when the clinical assignment letter
expires and there is a request from the medical committee; And
h. clinical authority recommendations and issuance of clinical assignment letters.
In carrying out the task of maintaining the professional quality of medical staff, the Medical
Committee performs the following functions:
a. implementation of medical audits;
b. recommendations for internal scientific meetings in the framework of
continuing education for medical staff;
c. recommendations for external activities in the context of continuing education
for the hospital's medical staff; And
d. recommendations for the proctoring process for medical staff in need'
In carrying out the task of maintaining discipline, ethics and professional behavior of medical
staff, the Medical Committee has the following functions:
a. fostering ethics and discipline of the medical profession;
b. examination of medical staff suspected of having committed a disciplinary
violation;
c. recommendations for disciplining professional actors in hospitals; And
d. giving advice or consideration in making ethical decisions on patient medical
care.
In addition to the Medical Committee, other committees can be formed to carry out certain
functions in the Hospital according to the needs and developments in science and technology
in order to improve the quality of service and patient safety.
Other committees in the form of committees:
a. nursing;
b. pharmacy and therapy;
c. infection prevention and control;
d. control of antimicrobial resistance;
e. ethics and law;
f. education coordination; And
g. risk management and patient safety.
The Medical Committee and other committees are carried out in accordance with statutory
provisions.
6. Internal inspection unit
The internal inspection unit is an organizational element in charge of carrying out
internal hospital performance audits. The internal inspection unit is under and responsible to
the head of the hospital or the director of the hospital.
In carrying out its duties, the internal audit unit carries out the following functions:
a. monitoring and evaluating the implementation of risk management in hospital
work units;
b. evaluation of the system of control, management and monitoring of the
effectiveness and efficiency of systems and procedures in the field of service
administration, as well as general and financial administration;
c. carrying out special tasks within the scope of internal control assigned by the
head of the Hospital or the director of the Hospital;
d. monitoring the implementation and accuracy of follow-up implementation on
the audit report; And
e. provision of consultation, advocacy, guidance, and assistance in implementing
hospital operational activities.
7. General administration and finance
The elements of general administration and finance are organizational elements in the
field of general administration and financial services which are under and are responsible to
the head of the Hospital or the director of the Hospital. The elements of general
administration and finance are led by directors, deputy directors, heads of departments or
managers. The elements of general administration and finance are tasked with carrying out
general and financial administration.
In carrying out general administration tasks, the elements of general administration and
finance carry out management functions:
a. administration;
b. housekeeping;
c. legal services and partnerships;
d. marketing;
e. public relations;
f. recording, reporting, and evaluation;
g. research and development;
h. human Resources; And
i. education and training.
In carrying out financial tasks, elements of general administration and finance carry out
functions:
a. budget planning;
b. treasury and fund mobilization; And
c. accountancy.

A. STRUKTUR ORGANISASI RUMAH SAKIT
Berdasarkan UU RI nomor 44 tahun 2009 pasal 33 tentang rumah sakit, setiap rumah
sakit harus memiliki organisasi yang efektif, efisien, dan akuntabel. Organisasi Rumah Sakit
paling sedikit terdiri atas Kepala Rumah Sakit atau Direktur Rumah Sakit, unsur pelayanan
medis, unsur keperawatan, unsur penunjang medis, komite medis, satuan pemeriksaan
internal, serta administrasi umum dan keuangan.

Jika disimpulkan maka Struktur Organisasi Rumah Sakit terdiri dari:

1. Kepala Rumah Sakit atau Direktur

Kepala Rumah Sakit atau direktur Rumah Sakit sebagaimana dimaksud adalah pimpinan
tertinggi dengan nama jabatan kepala, direktur utama, atau direktur. Kepala Rumah Sakit atau
direktur Rumah Sakit memimpin penyelenggaraan Rumah Sakit.
Dalam melaksanakan tugas, kepala Rumah Sakit atau direktur Rumah Sakit
menyelenggarakan fungsi:
a. koordinasi pelaksanaan tugas dan fungsi unsur organisasi;
b. penetapan kebijakan penyelenggaraan Rumah Sakit sesuai dengan kewenangannya;
c. penyelenggaraan tugas dan fungsi Rumah Sakit;
d. pembinaan, pengawasan, dan pengendalian pelaksanaan tugas dan fungsi unsur
organisasi; dan
e. evaluasi, pencatatan, dan pelaporan.
2. Unsur Pelayanan bidang Medis

Unsur pelayanan medis sebagaimana merupakan unsur organisasi di bidang pelayanan


medis yang berada di bawah dan bertanggung jawab kepada kepala Rumah Sakit atau
direktur Rumah Sakit. Unsur pelayanan medis dipimpin oleh direktur, wakil direktur, kepala
bidang, atau manajer. Unsur pelayanan medis bertugas melaksanakan pelayanan medis.

Dalam melaksanakan tugas, unsur pelayanan medis menyelenggarakan fungsi:


a. penyusunan rencana pemberian pelayanan medis;
b. koordinasi dan pelaksanaan pelayanan medis;
c. pelaksanaan kendali mutu, kendali biaya, dan keselamatan pasien di bidang pelayanan
medis; dan
d. pemantauan dan evaluasi pelayanan medis.

Unsur pelayanan medis meliputi pelayanan rawat jalan, rawat inap, dan gawat darurat.
3. Unsur Keperawatan

keperawatan merupakan unsur organisasi di bidang pelayanan keperawatan yang berada


di bawah dan bertanggung jawab kepada kepala Rumah Sakit atau direktur Rumah Sakit.
keperawatan dipimpin oleh direktur, wakil direktur, kepala bidang, atau manajer. bertugas
melaksanakan pelayanan keperawatan.

Dalam melaksanakan tugas, unsur keperawatan menyelenggarakan fungsi:


a. penyusunan rencana pemberian pelayanan keperawatan;
b. koordinasi dan pelaksanaan pelayanan keperawatan;
c. pelaksanaan kendali mutu, kendali biaya, dan keselamatan pasien di bidang
keperawatan; dan
d. pemantauan dan evaluasi pelayanan keperawatan.

4. Unsur Penunjang Medis

Unsur penunjang medis merupakan unsur organisasi di bidang pelayanan penunjang medis
yang berada di bawah dan bertanggung jawab kepada kepala Rumah Sakit atau direktur
Rumah Sakit. Unsur penunjang medis dipimpin oleh direktur, wakil direktur, kepala bidang,
atau manajer. Unsur penunjang medis bertugas melaksanakan pelayanan penunjang medis.

Dalam melaksanakan tugas unsur penunjang medis menyelenggarakan fungsi:


a. penyusunan rencana pemberian pelayanan penunjang medis;
b. koordinasi dan pelaksanaan pelayanan penunjang medis;
c. pelaksanaan kendali mutu, kendali biaya, dan keselamatan pasien di bidang
pelayanan penunjang medis;
d. pengelolaan rekam medis; dan e. pemantauan dan evaluasi pelayanan penunjang
medis.

Rumah Sakit dapat membentuk unsur pelayanan penunjang non medis sesuai dengan
kebutuhan. Kepala Rumah Sakit atau direktur Rumah Sakit menetapkan lingkup pelayanan
atau bidang yang masuk dalam unsur pelayanan penunjang medis dan unsur pelayanan
penunjang non medis.

5. Komite Medis

Komite Medis merupakan unsur organisasi yang mempunyai tanggung jawab untuk
menerapkan tata kelola klinis yang baik (good clinical governance). Komite Medis dibentuk
oleh dan bertanggung jawab kepada kepala Rumah Sakit atau direktur Rumah Sakit.
Komite Medis bertugas meningkatkan profesionalisme staf medis yang bekerja di
rumah sakit dengan cara:
a. melakukan kredensial bagi seluruh staf medis yang akan melakukan pelayanan medis di
rumah sakit;
b. memelihara mutu profesi staf medis; dan
c. menjaga disiplin, etika, dan perilaku profesi staf medis.

Dalam melaksanakan tugas kredensial, Komite Medis menyelenggarakan fungsi:


a. penyusunan dan pengkompilasian daftar kewenangan klinis sesuai dengan masukan
dari kelompok staf medis berdasarkan norma keprofesian yang berlaku;
b. penyelenggaraan pemeriksaan dan pengkajian kompetensi, kesehatan fisik dan
mental, perilaku, dan etika profesi;
c. evaluasi data pendidikan profesional kedokteran atau kedokteran gigi berkelanjutan;
d. wawancara terhadap pemohon kewenangan klinis;
e. penilaian dan pemutusan kewenangan klinis yang adekuat;
f. pelaporan hasil penilaian kredensial dan menyampaikan rekomendasi kewenangan
klinis kepada komite medik;
g. pelaksanaan proses rekredensial pada saat berakhirnya masa berlaku surat penugasan
klinis dan adanya permintaan dari komite medik; dan
h. rekomendasi kewenangan klinis dan penerbitan surat penugasan klinis.

Dalam melaksanakan tugas memelihara mutu profesi staf medis, Komite Medis
menyelenggarakan fungsi:
a. pelaksanaan audit medis;
b. rekomendasi pertemuan ilmiah internal dalam rangka pendidikan berkelanjutan bagi
staf medis;
c. rekomendasi kegiatan eksternal dalam rangka pendidikan berkelanjutan bagi staf
medis rumah sakit tersebut; dan
d. rekomendasi proses pendampingan (proctoring) bagi staf medis yang membutuhkan’

Dalam melaksanakan tugas menjaga disiplin, etika, dan perilaku profesi staf medis,
Komite Medis menyelenggarakan fungsi:
a. pembinaan etika dan disiplin profesi kedokteran;
b. pemeriksaan staf medis yang diduga melakukan pelanggaran disiplin;
c. rekomendasi pendisiplinan pelaku profesional di rumah sakit; dan
d. pemberian nasehat atau pertimbangan dalam pengambilan keputusan etis pada asuhan
medis pasien.

Selain Komite Medis, dapat dibentuk komite lain untuk penyelenggaraan fungsi
tertentu di Rumah Sakit sesuai kebutuhan dan perkembangan ilmu pengetahuan dan teknologi
dalam rangka meningkatkan mutu pelayanan dan keselamatan pasien.

Komite lain berupa komite:


a. keperawatan;
b. farmasi dan terapi;
c. pencegahan dan pengendalian infeksi;
d. pengendalian resistensi antimikroba;
e. etika dan hukum;
f. koordinasi pendidikan; dan
g. manajemen risiko dan keselamatan pasien.

Komite Medis dan komite lain dilaksanakan sesuai dengan ketentuan peraturan perundang-
undangan.

6. Satuan pemeriksa Internal


Satuan pemeriksaan internal merupakan unsur organisasi yang bertugas melaksanakan
pemeriksaan audit kinerja internal rumah sakit. Satuan pemeriksaan internal berada di bawah
dan bertanggung jawab kepada kepala Rumah Sakit atau direktur Rumah Sakit.
a. Dalam melaksanakan tugas, satuan pemeriksaan internal menyelenggarakan
fungsi:
b. pemantauan dan evaluasi pelaksanaan manajemen risiko di unit kerja rumah
sakit;
c. penilaian terhadap sistem pengendalian, pengelolaan, dan pemantauan
efektifitas dan efisiensi sistem dan prosedur dalam bidang administrasi
pelayanan, serta administrasi umum dan keuangan;
d. pelaksanaan tugas khusus dalam lingkup pengawasan intern yang ditugaskan
oleh kepala Rumah Sakit atau direktur Rumah Sakit;
e. pemantauan pelaksanaan dan ketepatan pelaksanaan tindak lanjut atas laporan
hasil audit; dan
f. pemberian konsultasi, advokasi, pembimbingan, dan pendampingan dalam
pelaksanaan kegiatan operasional rumah sakit.

7. Administrasi umum dan keuangan


Unsur administrasi umum dan keuangan merupakan unsur organisasi di bidang pelayanan
administrasi umum dan keuangan yang berada di bawah dan bertanggung jawab kepada
kepala Rumah Sakit atau direktur Rumah Sakit. Unsur administrasi umum dan keuangan
dipimpin oleh direktur, wakil direktur, kepala bidang, atau manajer. Unsur administrasi umum
dan keuangan bertugas melaksanakan administrasi umum dan keuangan.
Dalam melaksanakan tugas administrasi umum unsur administrasi umum dan
keuangan menyelenggarakan fungsi pengelolaan:
a. ketatausahaan;
b. kerumahtanggaan;
c. pelayanan hukum dan kemitraan;
d. pemasaran;
e. kehumasan;
f. pencatatan, pelaporan, dan evaluasi;
g. penelitian dan pengembangan;
h. sumber daya manusia; dan
i. pendidikan dan pelatihan.

Dalam melaksanakan tugas keuangan, unsur administrasi umum dan keuangan


menyelenggarakan fungsi:
a. perencanaan anggaran;
b. perbendaharaan dan mobilisasi dana; dan
c. akuntansi.

Closing
a. Consulusion
Whatever and however, the hospital is the right place for people who experience
health problems, both mental, physical and others. even though there is a home care system
that is carried out by some people, it is still not optimal when compared to the care system
that has been carried out in every hospital. Classification and Organizational Structure of
Hospitals are specific standard provisions that must exist in every hospital, whether private or
managed by the Government.
Kesimpulan
Apapun dan bagaimanapun, Rumah Sakit merupakan tempat yang tepat orang -orang yang
mengalami gangguan kesehatan, baik jiwa, fisik dan lainnya. walaupun ada sistem perawatan
rumah yang dilakukan oleh sebahagian orang, namun tetap saja tidak maksimal jika
dibandingkan dengan sistem perawatan yang telah dilakukan di setiap Rumah Sakit. Klasifikasi
dan Struktur Organisasi Rumah Sakit merupakan standart ketentuan khusus yang harus ada
pada tiap-tiap rumah sakit, baik itu swasta ataupun yang dikelola oleh Pemerintah.
