1. SCOPE AND PURPOSE

The purpose of this document is to establish the requirements for the use of lifting equipment on site.
It applies to all Cranes, Truck Cranes and Rigging Devices.

PROJECT common definitions are listed in the F12254-CDB-W1-PMT-PLA-00001 - Project Execution
Plan.
Rigger – an experienced and specialized person with knowledge and safety skills on the lifting and
moving of extremely large and heavy objects with the use of heavy equipment.
Banksman – a person with knowledge in International Standard Signal (See 4.1.4) and safety
procedures regarding load handling.

4. ACTIVITIES DESCRIPTION

4.1 GENERAL PROCEDURE


Depending on the weight of the load to be lifted, the load is classified as following:
- LIGHT LIFT
ƒ Load Weight <= 20.000kg
ƒ Load Weight <= 80% Load Chart Capacity
ƒ Required document: No formal data sheet or rigging plan required
- MEDIUM LIFT:
ƒ 20.000kg < Load Weight <= 50.000kg
ƒ 80% Load Chart Capacity < Load Weight <= 90% Load Chart Capacity
ƒ Required document: Rigging Plan shall be signed by CDB Rigging Supervisor
and by Owner representative
- HEAVY LIFT:
ƒ Load Weight > 50.000kg
ƒ 80% Load Chart Capacity < Load Weight <= 90% Load Chart Capacity

- CRITICAL LIFT:
ƒ Load Weight > 90% Load Chart Capacity
ƒ Multiple-Crane lift where either Load Weight > 80% Chart Capacity or a
change of location of crane is required
ƒ Required document: Rigging Plan shall be signed by CDB Rigging Supervisor
and by Owner representative.

The Rigging Plan shall contain the following information:


• Weight of load.
• Radius of operation.
• Crane capacity for that radius.
• Interferences
• Lifting devices
• Supporting drawings
• Data sheets
• Travel route and rigging documentation

See Appendix B – Rigging Plan for more details.

4.1.1 Operator
The crane shall be operated only by the following qualified personnel:
• Designated operators - certification or past proved experience records shall be
available on equipment department on site.
• Trainee Operator with past experience in heavy crane operation, but with no
experience in a specific crane model. In this case, the Trainee Operator shall always
be under experimented operator supervision until the experimented operator certifies
that the Operator is capable to operate without supervision.
• Maintenance personnel including inspector qualified to operate the crane, when
performing their duties.
No other personnel are allowed to operate any crane.
During lubricating and greasing activities no lifting activity can be done with the crane.

4.1.2 Working Area


The following general procedures will be taken under account in order to perform a safe
lift:
• The radius area inside of the crane working area should be isolated, and only authorized
personnel will be working inside this area. The working area will be isolated accordingly
with the crane working radius. A warning signage will be posted at the border of the
isolated area. This sign will say: CRANE IN OPERATION – LIFTED LOADS – KEEP
DISTANCE.

• It is prohibited to work or stand underneath a suspended load


• It is prohibited to lift loads during a windy day. Booms shall be lowered, if possible, under
high wind conditions. Wind velocity will be measured by a calibrated anemometer. Each
crane operator shall check the manufacturer’s recommendations prior to operating the
crane in windy conditions. The anemometer will be installed on site and, in case the wind
speed reach 50 km/h (30 mph), an alert will be issued to suspend all hoisting operations.
• Whether conditions (fog, wind and occasional rain) must be considered by the rigger
before authorizing the lift.
• Prohibited to lower the outriggers on the step walkway of the trestle.
• Night activities will be only allowed after a good illumination, in order to provide good
visibility and communication between crane operator and helper (on site signal
responsible)
• Night activities shall be approved by a contractor manager and a Work Permit shall be
required.
• Delimitation of the lifting area must be installed (safety yellow tapes and proper
signalization will surround the area attached to grounded poles) and supervised by a
foreman. Worker must be also responsible for the strings to guide.
• For lines rated 50kV or less, the minimum clearance between the lines and any part of
the crane or load shall be 3m. For higher voltages a study shall be done.
• Crane must be placed in a strategic position, next to the structured to be assembled,
respecting its force angles and movements.
• Ground consideration shall be taken under account to ensure stability of the crane
during operation.

4.1.3 Handling the load

4.1.3.1 Size of Load


• Payload Weight: The weight of the item to be lifted. Payload weight includes the load
weight, plus the weight of attachments, saddles, etc.
• Before lifting a load which weight is unknown, the engineer or superintendent in charge
of the work should assess the estimated weight of the load and verify that the weight of
the load is within the static or dynamic rating for the radius at which the load will be lifted.
• The crane shall not be loaded beyond the applicable static or dynamic ratings given on
the capacity chart for the crane. All cranes will have devices to measure the load and a
shutdown system if it is loaded beyond its limits.
• For loads greater than 20.000kg a Rigging Study shall be signed by the engineer in
charge of the lifting.
• The capacity charts for the crane shall be kept in the operator’s cab.

4.1.3.2 Attaching the Load

• Attach the hook to the load with nylon slings or polyester slings. Each hook shall have a
latch that is in proper working order. Hook latches shall not be wired open.
• Only use slings and other rigging devices which are in safe operating condition (after it
has been inspected and approved by the foreman in charge) and have a rating equal to
or greater than the load to be lifted.
• Do not wrap the load line around the load.

• Use clamps to kill the ‘live’ edges for protection of the slings from any sharp edges on
the load.
• Secure the unused legs of a multi-leg sling before handling a load with one leg of the
sling.

4.1.3.3 Lifting/Moving the Load

Before lifting or moving the load, the operator or the designated person directing the lift
shall make the following checks:
• The supporting surface under both crawlers, all tires, or each outrigger jack pad or
float crane is uniform an firm. Unless otherwise specified on the capacity chart, the
footing shall be level to within 1% (one meter rise or fall in 100 meter distance).
• After the load is secured and properly balanced in the slings or lifting device, the
Rigger shall perform a visual inspection. Then the load shall be lifted a few inches and
the Rigger shall perform another visual inspection. If everything is OK, than the Rigger
will give the final authorization to proceed with the operation.
• Slings dimension will be according to the load to be lifted. Steel cables, slings, etc
will have a minimum safe factor of five (05). Every steel cable will be inspected
monthly, after a long period without usage or damage notification. Check Lifting and
Rigging Equipment Inspection Procedure for wire ropes inspections and substitution.
• The lift and swing paths are clear of personnel and obstructions.
• The load is free to be lifted.
• The load line is not kinked or otherwise damaged.
• Multiple part load lines are not twisted around each other in such a manner that the
line will not separate when the load is lifted.
• The hook is brought over the load in a manner that will minimize twisting or
swinging.
• The load line and boom hoist ropes are properly and evenly spooled on the drums
and seated in the sheaves.
• The operator shall test the brakes each time a load approaching the rated load is
handled by lifting the load a few inches and applying the brakes.
• While lifting or moving the load, the operator shall take the following precautions:
o Accelerate and decelerate the load smoothly to keep it from jerking the load
around and possibly damaging it and also to avoid excessive stress on the crane
boom and machinery
o Avoid sudden starts and stops while swinging. Keep the swing speed under
control to prevent the load from swinging out beyond the radius at which the load
can be handled and to minimize the pendulum action of the load.
o Use taglines or other restraints to control the load when necessary.
o Do not exceed any swing limitations (areas of operation) given on the capacity
chart.
o Do not allow the load, the boom, or any other part of the crane to contact
obstructions.
o Do not use the crane to drag a load.
o Do not hoist, lower, or swing the load while personnel are on the load or hook.

o Avoid carrying the load over personnel. Loads which are suspended shall be
blocked or cribbed before personnel are allowed to-work under or between
them.
• Before lifting a load which requires the use of outriggers (or anytime outriggers are
used), fully extend the outrigger beams and jacks so the truck tires do not bear any
load.
• Securely fasten outrigger jack pads or floats to the jacks and set them on a flat, firm
surface that will support the load placed on the pads or floats. Do not set jack pads or
floats in holes, on rocky ground, or on extremely soft ground.
• When dictated by ground conditions, install wood blocking or steel plates under the
jack pads or floats to properly distribute loading on the supporting surface.
• Wood blocking or steel plates used under the jack pads or floats shall be:
o Free of defects.
o Strong enough to prevent crushing, bending or shear failure.
o Of sufficient thickness, width, and length to completely support the jack pad or
float, transmit the load to the supporting surface, and prevent shifting, toppling,
or excessive settlement under load.
• The Rigger shall be responsible for the operation when two or more cranes are
used to lift the same load. The Rigger shall analyze the lift and instruct all personnel
involved in the proper rigging and positioning of the load and all movements to be
made. Decisions such as the necessity to reduce crane ratings, load position, ground
support, and speed of movements shall be in accordance with the Rigging Study and
be approved by the superintendent in charge of the operation.
• Do not lower the load to the point that less than two full wraps of wire rope remain
on the respective drum.

While travelling, the operator shall take the following precautions:


• Carry the boom in line with the lower works and facing the direction of travel.
• Lock the upper works against rotation except when it is necessary to negotiate a
turn, and then only when the operator is seated at the controls or the boom is
supported on a dolly.
• Lash or otherwise restrain unused hooks so they cannot swing freely.

Before travelling with a load, the operator shall take the following additional
precautions:
• The Rigger shall be responsible for the operation. Decisions such as the necessity
to reduce crane ratings, load position, ground support, and speed of movements shall
be in accordance with the designated person’s decision
• Maintain specified tire pressures (truck cranes).
• Avoid sudden starts and stops. Use taglines or other restraints to control the
position of the load.
• Fully retract and lock the jacks and outrigger beams so they cannot extend when
not in use.

4.1.3.4 Holding the Load

When a load is suspended, the operator shall take the following precautions:

• Not leave his/her position at the controls.


• Not allow personnel to stand or pass under the load.
• Move all controls to off, apply all drum brakes, engage the boom hoist pawl, and
apply the swing and travel brakes or locks.

4.1.4 Signals
Signals to the operator shall be in accordance with the standard signals unless
communications equipment (telephone, radio, etc. is used). The International Standard
Signal will be used see drawing attached below:

All signals shall be easily understood by the operator at all times. The operator shall not
respond to any signals which are not clearly understood. The operator shall only take
direction from the designated Rigger responsible for signalling.
Additional signals may be required. In those cases, the signals used shall be agreed
upon in advance by the operator and the signal person. The signals used shall not
conflict with or have the potential to be confused with the standard signals.
When it is necessary to give instructions to the operator (other than those established by
the signal system), all crane motions shall be stopped.

The Rigger shall:


• Be qualified and have proven experience in crane operations and be thoroughly familiar with the
International Standard Signals.
• Be positioned in clear view of the operator. The banksman position should give him or her clear
view of the load, the crane, and the operating area.
• Direct the load so the load does not pass over personnel.
• Keep unnecessary personnel out of the crane’s operating area.
• All the accessories and materials used during crane lifting of loads must have a specialized and
qualified banksman, who will use the standard signs and radio communication (difficult visualization
points). There must be a coordination meeting between the banksman and crane operator before
every lift.
• Audible and visual signaling chart should always be in presence of crane operator, banks man and
rigger.

4.1.5 Daily Inspection


Crane operator must first make a visual inspection on his equipment before starting any
activity. Upon observing any abnormality, the operator shall immediately inform the
responsible maintenance personnel and safety engineer. In case of defective equipment,
it will be identified, isolated and removed from working site for revision accordingly with
‘F12254-CDB-W01-EQP-PRO-00755 Lifting and Rigging Equipment Inspection’
procedure.
The following items must be checked:
• Cables
• Electric system
• Engine
• Wheels / tracks
• Command system
• Hydraulic System,
• Oil leaks and drips
• Rigging Devices
See Appendix C for more details.

4.1.5.1 Cable Inspection Control


An adequate wire rope inspection procedure is the most important action for an adequate
and safe lifting. Wire Rope Inspection procedure presented by a membership between

the Crane Manufacturers Association of America, Inc. (CMAA) and OSHA.


This procedure outlines the visual inspection needed to ensure the integrity of wire rope
used with overhead and gantry cranes installed. Implementing this procedure is vital to
personnel and equipment safety and satisfies industry-standard requirements for the
periodic inspection of running ropes on overhead and gantry cranes. This policy shall also
be applied to other material handling equipment where wire rope is used lifting devices
and vehicle mounted cranes.
Inspections will be conducted (at a minimum) monthly. Only qualified persons shall
inspect the cables.
Inspection Procedure

1. Lower the hook to a position immediately above the floor.


2. Inspect the entire length of the wire rope for signs of deterioration (broken wires,
worn outside wires, reduction in diameter, stretch, corrosion, lubrication and heat
damage. Also, inspect for signs of distortion on the rope, to include: crushing and
flattening, cuts, kinks or bends, unstranding, bird caging, main strand
displacement and core protrusion. Notify the safety engineer if you detect signs of
deterioration or defects. The safety engineer will make a determination on
continued use of the rope.
3. Inspect the rope lay on the hoist drum.
4. Inspect the hook and the sheaves of the hook block.
5. Raise the hook to its stowed position (or its safest height)
6. Complete and sign the inspection report. (See Appendix A). This report must
include:
- Date of inspection
- Signature of person who performed the inspection
- Identification of the rope inspected

Every cable after monthly inspection will be identified according to the monthly colors
code shown below:

If cables are not tag colored accordingly with the table above they will be identified,
isolated and removed from working area.

A visual inspection will also be performed daily to detect any damage on the cables that
could cause any risk during lifting. If any noticed insecurity, the Rigger must inform the
Superintendent and the cable must be inspected by the safety engineer.

4.1.6 Equipment Inspection and Maintenance


All cranes on site will be inspected and maintained according with manufacturer
procedures and as seen on the document ‘F12254-CDB-W01-EQP-PRO-00755 Lifting
and Rigging Equipment Inspection’ and the HSE Inspection and Maintenance of tools and
equipment.

4.1.7 Crane erection


Before any crane erection, it is important to have available at job site the procedure for
each crane assembling. Furthermore, all lifting activities shall be evaluated in detail in
order to define the correct auxiliary crane that shall be used to support the assembling
activity.
All crane operations will be directed by, a single authorized, clearly identified rigger (will
have a yellow vest). Travel of mobile cranes on site shall have a banks man provided as
a walking escort.
Where line of sight is not possible, two-way radios shall be used.
All two-way radios are to be checked prior to any work commencing.
Constant communication is to be maintained at all times when line of sight is obscured.
At any time communication is unclear or lost, all crane movements are to cease
immediately.
The load shifting operator and banksman are to satisfy themselves that crane capacity
and set out of operations is acceptable and to manufactures specifications. A construction
engineer can be used as a reference to double check calculations as required.
The area within the swing radius of cranes shall be barricaded and warning signs erected
where personnel could inadvertently enter the lift zone.
All Load Shifting Operators and riggers shall observe manufacturer’s specifications during
crane and rigging operations.
Rated load capabilities, recommended operating speeds, special hazard warning and
special instructions shall be visible to the Operator cabin.
Employees shall not be allowed to work under the load of cranes.
Tag lines shall be used to maintain control of suspended loads.
Cranes shall be inspected by qualified and certified personnel prior to use (check if any
damage between Callao Port and Melchorita Site) and at monthly intervals. The person
performing the inspection shall maintain records in the crane logbook.

4.1.8 Lifting equipment used in crane erection


The lifting equipment used in crane erection activity shall be evaluated in function of the
loads of the crane parts to be assembled, considering all safety measures needed in
lifting activities
All lifting equipment used in the crane erection activity may be inspected as indicated in
the ‘F12254-CDB-W01-EQP-PRO-00755 Lifting and Rigging Equipment Inspection’,
furthermore:
• All items of equipment to be used in the lift shall comply with manufactures
requirements and specifications on their intended use.

• Certificates will be provided for all cranes prior to use.


• For items that do not require certification, the construction Engineer must ensure that
calculations demonstrating the load-bearing capacity of the item, including lifting
slings, brackets, attachments etc. are submitted for approval in accordance with any
statutory requirements. Where Pad eyes or lifting lugs are used as lifting points a
NDT inspection shall be carried prior to use and every two years.

Preparation for Crane Lift


The Manager/Superintendent shall require the respective construction Engineer or Rigger
to verify the condition of the item to be lifted to ensure the conditions set out in the
Rigging Study and risk analysis remain unchanged.
The verification shall consider, as a minimum; the dimensional data compared to the
vendor-certified drawing included in the study, that no additional items have been added
to the equipment, which, may not have been considered by the study. e.g. insulation,
platforms and ladders, internals, and the weight of the item shown on the final
documentation. This review should also factor in site condition changes and
environmental conditions (wind, rain, etc).
Prior to the lift being undertaken, the Supervisor shall ensure that each item of rigging
equipment, including crane ropes, blocks, drums and brakes, are in good working order
and condition and have been visually inspected by the Crane Operator/Rigger.
All personnel involved in a significant lift shall participate in a Job Safety Analysis session
immediately prior to the lift being undertaken.
No crane lift shall be undertaken, if in the opinion of the Supervisor, Crane Operator,
Safety Engineer, banksman or Construction Engineer, the weather conditions could
potentially cause an unsafe condition.
Under no circumstances shall any load be left suspended or UN-supervised, pending
improvement of weather conditions or overnight in the event the lift is interrupted.
All rigging equipment, slings, shackles, hooks and ropes must be free from defects and in
good operating condition and shall be subject to a monthly documented inspection.
All rigging equipment shall have as a minimum, manufactures test certification and where
this is not proven the project must ensure that the equipment is tested by an authorized
body before being put into service.
The CONTRACTOR Construction Manager shall delegate the responsibility of
maintaining records of equipment certificates, and inspection details to a competent
person.
Equipment inspection records shall as a minimum identify the type, size, brand, and
unique number, date of inspection and name of the competent person performing the
inspection.
All lifting and rigging equipment shall be contained in a secure storage, under inventory
control and issued under the charge of a competent person. This shall include visual
inspection after and before return to use.

4.1.9 Personnel Platform Specifications


• Personnel Platform shall be designed by a qualified engineer, company or a qualified
person competent in structural design.
• Platform shall have anchorage points for a fall arrest system.
• A grab rail shall be installed inside the entire perimeter of the personnel platform.

• All welding of the personnel platform and its components shall be performed by a
qualified welder familiar with the weld grades, types and material specified in the
platform design.
• All rough edges exposed to contact by employees shall be surfaced or smoothed in
order to prevent injury to employees from punctures or lacerations.
• The personnel platform shall be conspicuously posted with a plate or other permanent
marking its rated load capacity or maximum intended load.
• Access gates, including sliding or folding gates, shall be equipped with a restraining
device to prevent accidental opening.
• Access gates, if installed, shall not swing outward during hoisting.
• Each personnel platform shall be equipped with a guardrail system and shall be
enclosed at least from the toe board to mid-rail with either solid construction or
expanded metal having openings no greater than \1/2\ inch (1.27 cm).
• In case of existing risk of falling objects, provisions for overhead protections will be
taken.

4.1.9.1 Worker inside Platform


• Workers (electrician, mechanic, welder, etc) must be qualified in his area of work
and must know all the procedure before performing any type of work inside the
platform.
• Personnel occupying the Personnel Platform shall use a body harness system with
lanyard appropriately attached to the lower load block on the overhaul ball.
• Personnel being hoisted shall remain in continuous sight of and in direct
communication with the operator or signal person.
• The number of employees occupying the personnel platform shall not exceed the
number required for the work being performed or the number that the platform is
certified for.

4.1.9.2 Size of Load


• The man platform shall have a permanently affixed plate which indicates the
weight of the man platform and its rated capacity.
• The operator or the designated person directing the platform shall verify that the
weight of the load is within its capacity.
• The load line of the crane must have a minimum safety factor of seven and must
have a rotation-resistant rope with a minimum safety factor of ten.
• Combined weight of the loaded personnel platform and its rigging must not exceed
50% of the crane capacity or the radius and configuration of the crane.

4.1.9.3 Attaching the Platform


After trial lift the Personnel Platform shall be inspected to ensure that it is secure and
properly balanced. Personnel shall not be hoisted unless the following conditions are
met:
ƒ Hoist ropes shall be free of kinks
ƒ The primary attachment shall be centered over Personnel Platform

ƒ Hoisting rope shall be properly fitted on drum and sheaves

4.1.9.4 Lifting/Moving the Personnel Platform


Before lifting or moving the load, the operator or the designated person directing the
lift shall make the following checks:
ƒ The crane has a firm uniformly supporting surface under both crawlers,
all tires, or each outrigger jack pad or float. Unless otherwise specified
on the capacity chart, the footing shall be level to within 1%.
ƒ The person is secured and properly balanced before lifting the platform.
ƒ The lift and swing paths are clear of personnel and obstructions.
ƒ The hook is brought over the load in a manner that will minimize twisting
or swinging.
ƒ The load line and boom hoist ropes are properly spooled on the drums
and seated in the sheaves.
ƒ The operator shall test the brakes each time a load approaching the
rated load is handled by lifting the load a few inches and applying the
brakes.

While lifting or moving the platform, the operator shall take the following precautions:
ƒ Accelerate and decelerate the load smoothly to avoid excessive stress
on the crane boom and machinery.
ƒ Hoisting of the Personnel Platform shall be performed in a slow,
controlled, cautious manner with no sudden movements of the crane.
ƒ Do not exceed any swing limitations (areas of operation) given on the
capacity chart.
ƒ Personnel shall keep all parts of the body inside the Personnel Platform
during raising and lowering.
ƒ The crane MUST not move while working with the personnel platform

4.1.9.5 Holding the Personnel Platform.


When a load is suspended, the operator shall take the following precautions:
• Not leave his/her position at the controls.
• Not allow personnel to stand or pass under the platform.
• Move all controls to off, apply all brakes, engage the boom hoist pawl, and apply
the swing and travel brakes or locks.
• The crane operator must always be at the controls when the crane engine is
running and the personnel platform is occupied. The crane operator also must
have full control over the movement of the personnel platform.

4.1.10 Safety
Every lifting service must follow this procedure, combined together with the Preliminary
Working Analysis (PWA), properly signed by all the responsible for this activity, and
participate in the HSE daily meeting.

The access to the trestle will be allowed to authorized personnel only. A person in
charge of Operation and a HSE team member must always be present when lifting
heavy equipments.

4.1.11 Risk analysis


A risk analysis shall be done using the Hazard Identification Plan (F12254-CDB-W01-
HSE-PLA-00115) by the Supervisor that will be in charge of the erection activity and a
copy of this analysis shall be kept in the equipment file and another shall be kept on
site by the task supervisor.
