Osteomalacia & Rickets

Osteomalacia is the softening of the bones caused by impaired bone
mineralization primarily due to inadequate levels of available vitamin D, calcium and
phosphate.
The most common cause of osteomalacia is a deficiency of vitamin D, which is
normally derived from sunlight exposure and, to a lesser extent, from the diet.

Osteomalacia in children is known as rickets)‫(الكساح‬, and the term
"osteomalacia" is often restricted to the adult type.

Signs and symptoms:

 Diffuse "generalized" body pains (bone & joint pain).
 Muscle weakness.
 Easy fracturing.

compensatory osteoblast activity
The most specific diagnostic test is a serum 25(OH)D level in blood.
Then, low serum and urinary calcium, Low serum phosphate, Elevated serum alkaline
phosphatase (due to an increase in compensatory osteoblast activity).

Vitamin D and calcium supplements, Vitamin D should always be administered
in conjunction with calcium supplementation and sun exposure.

.OTC ‫دا بيطلع‬ Vitamin D3 (cholecalciferol)
Vitamin D3 (cholecalciferol) is typically absorbed more readily than vitamin D2

 Rheumatoid Arthritis

Introduction:
Rheumatoid arthritis (RA): is a complex systemic inflammatory autoimmune
condition manifesting initially as symmetric swollen and tender joints of the hands
and/or feet.

Some patients may experience low disease activity, whereas others may
present with high disease activity and/or extra articular manifestations.
The systemic inflammation of RA leads to joint destruction, disability, and
premature death.

Epidemiology:
RA has a prevalence of 1%.

- Female gender (3:1 females to males).
- Increasing age (peak onset 35-50 years of age).

- Stress.

Pathophysiology:

2. Destruction or damage in bone matrix.
3. Reduced joint space.
4. Increased synovial fluid.

Activation of mature T lymphocytes requires two signals.

The first is the presentation of an antigen by antigen-presenting cells to the T-
lymphocyte receptor.
Second, a ligand-receptor complex (ie, CD80/CD86) on antigen-presenting cells binds to
CD28 receptors on T lymphocytes.

Once a cell successfully passes through both stages, the inflammatory cascade is
activated.

Activation of T lymphocytes:
stimulates the release of macrophages or monocytes, which subsequently causes
the release of inflammatory cytokines; (b) activates osteoclasts; (c) activates release of
matrix metalloproteinases or enzymes responsible for the degradation of connective
tissue; and (d) stimulates B lymphocytes and the production of antibodies.

 An imbalance of proinflammatory and anti-inflammatory cytokines in the

synovium leads to inflammation and joint destruction.
 Some of the proinflammatory cytokines are IL-1, TNF-α, IL-6, and IL-17.
 Anti-inflammatory cytokines and mediators (IL-4, IL-10, and IL-1 receptor
antagonist) are present in the synovium, although concentrations are not high
enough to overcome the effects of the proinflammatory cytokines.

B Lymphocytes:

Rheumatoid factors: are not present in all patients with RA, but their presence is
indicative of disease severity and structural progression.
highly specific ‫ لذلك اقدر اعتمد عليه اكتر ك‬، ACPA ‫ عندهم ال‬، RA ‫ معظم المرضى اللي عندهم‬
.highly confirmed diagnosis ‫أو‬
.aggressive disease ‫ مؤشر على ان فيه‬high level ‫ ب‬ACPA ‫ وجود ال‬
ACPA: are produced early in the course of disease.
High levels of ACPA are indicative of aggressive disease and a greater likelihood of poor
outcomes.

Kinases
 Are enzymes involved in communication or signaling activities within and
between cells.
 Activated kinases lead to cell activation and proliferation.
 Examples include Janus-associated kinases (JAKS) and spleen tyrosine kinase.

Comorbidities Associated with RA

 The comorbidities with the greatest impact on morbidity and mortality associated
with RA are:

 a. Cardiovascular disease, Low-dose aspirin, omega-3 fatty acids, statins, or
combination therapy should be considered.
b. In a patient with CHF, it is recommended to avoid TNF inhibitors.
c. Infections
d. Malignancy (GI cancers and lymphoproliferative disorders), and
e. Osteoporosis.

Diagnosis
 Both osteoarthritis and RA are prevalent in the US population, but they differ in
presentation.
