Tejeros, Makati City, Philippines 1204 after having been duly sworn to in accordance with law,
hereby depose and state.

1. That I am the Treasurer of Caledonian International Corporation with shipment 1 PK

Laser Tube as per Flight No. LD 0457 which arrived last March 20, 2021 with reg. no.
LDA 0068-21 and Airway Bill of Lading No. 7730752363.

2. That we failed to file the goods declaration within the prescribed period due to Late
arrival of shipping documents.

3. That I am requesting your good office for continue processing of our entry.

4. That should the Commission on audit (COA) disallow the release of the shipment later
on, we undertake to assume full responsibility for whatever financial obligation such
release of shipments may entail to the exclusion of the Bureau of Customs, particularly
present Collector of Customs.
