CIRCULAR

Sub: Issuance of ldentity Cards to Pensioners.

With the approval of competent authority, Pension Trust intend to start the
process of issuance of New Plastic identity cards to Pensioners. In order to
apply for the same, Pensioners are required to submit TWO sets of Proforma
with relevant details alongwith photograph affixed on it preferably in typed
format, in CAPITAL letters. The proforma with requisite details should be
submitted alongwith aself attested copy of PPO.

The already issued ldentity cards should be surrendered at the time of receipt
of new Cards.

Pensioners are also advised not to rush for submission of forms, as at present
no last date has been fixed for the same.
