RESOLUTION NO. 19-A

Series of 2021

THE SANGGUNIANG BARANGAY OF SALVACION SANTO TOMAS, DAVAO DEL NORTE WILL CHANGE
EXISTING ACCOUNT NAME AT DAVAO LIGHT AND POWER COMPANY, INC. TO THE NEW ACCOUNT
NAME OF “BARANGAY SALVACION” AND AUTHORIZING LORNA P. MADAYAG TO TRANSACT THE
PROCESS OF CHANGING ACCOUNT NAME.

WHEREAS, that the BIR ruling pursuant to section 113 (B) of the Tax Code , as amended and
Section 4, 113-1 (B) of RR No. 16-2005 and reiterated in Revenue Memorandum Circular No. 64-2015
which states that the following information must reflect on Receipts/Invoices/other commercial invoices
generated from cash registered machine;

WHEREAS, that the Account ID Number :7477312222-6 with Account Name of Kalahi Center will
Change to Account Name of “BARANGAY SALVACION”.

WHEREAS, to avoid BIR from disallowing taxes withheld, it is important that the correct
information of name, address, TIN and business style/reflected in the billing statement, tendered
through BIR Certificate (Form 2307) and the officials receipts (OR) issue must reflect the same
information indicated to entity’s BIR Registration;

WHEREAS, LORNA P. MADAYAG, appointed as Barangay Treasurer authorized to transact all

pertinent documents relevant to process of changing account name.

WHEREFORE, Be it Resolved, as it is Hereby Resolved by the Sangguniang Barangay of Salvacion,

Santo Tomas, Davao del Norte in a Regular Session Assembled to change existing Account Name at
Davao Light & Power, Co.,Inc. to new account name of “Barangay Salvacion” and authorizing Lorna P.
Madayag to transact the process of changing account name.
 RESOLVED FURTHER, that copies of this resolution be forwarded to the Office of the Davao Light
& Power Co.,Inc. –Panabo City, Davao del Norte Branch for proper information and appropriate action.

ADOPTED AND PASSED this 15th of June 2021.
