SUBJECT: INCIDENT REPORT, RE: SCBA Refiller and SCBA Units
DATE: 10 March 2024
OOA 10 0930H March 2024, while attempting to refill empty SCBA Cylinders with compressed air using the SCBA refiller donated by the City Government of Muntinlupa, duty SRF
personnel of Muntinlupa City Fire Station has noticed that the SCBA refiller was not properly functioning. A few seconds after turning the machine on, the SCBA refiller started to leak air from
one of its filters. The machine also then started making exploding noises from its other filter as if it was displacing pressurized air from inside. It has been also found out that it was not refilling
the SCBA cylinders with compressed air. The machine was immediately turned off to prevent further untoward incidents.
After turning the machine off, Duty SRF personnel conducted a visual check of the machine to see if there were any broken parts, but none was found. The engine oil, which can
be seen from its window, was also at sufficient level. The relief valves were also opened to release pressurized air from the machine. However, after doing basic troubleshooting of the machine,
the machine was still making the unnecessary noises and wasn’t able to fill the SCBA cylinders with compressed air.
No other checks were conducted afterwards for doing so may cause further problems, and disassembly of the machine may void any warranties the machine may have.
Furthermore, during the said activity, it has also been found out that two (2) Lung Demand Valves of the current SCBA units of SRF Muntinlupa have leaks, which makes the units
unserviceable. Basic troubleshooting was conducted by the duty SRF personnel, and upon checking, one of the LDVs has a broken piece inside the mouth piece, and the other’s locking
mechanism was loose.
Hence, SRF Muntinlupa humbly recommends to have the refiller brought back to Muntinlupa City Disaster Risk Reduction Management Office to be looked upon and to conduct
the repair. Unfortunately, for the Lung Demand Valves, there were no viable options as to where the said units might be brought for repair.
