I, DANILO BEHAY ETA, of legal age, Filipino Citizen, with present address #8 Sultana
Rd. Manuyo 1, Las Piñas City, after having been duly sworn to according to law, hereby depose
and state:

1. That I am the biological father of JAMES RYAN ALLERA LAGUNA, who

was born out of wedlock on November 15, 2001 at Fabella Memorial Hospital
Sta. Cruz, Manila City, out of my common-law-relationship with her mother,
THELMA L. ETA;
2. That my said child’s birth is duly registered with the Local Civil Registrar Office
of Manila City, under Registry No. 2001-107232, by the name JAMES RYAN
ALLERA LAGUNA;
3. That I hereby admit and acknowledge that i am the father of JAMES RYAN
ALLERA LAGUNA;
4. That I hereby agree and conform for my said child to carry my surname ETA;
5. That I am executing this affidavit in order to establish the foregoing sworn
statement and to attest to the truth thereof.
