AFFIDAVIT OF LOSS

I, ARSENIO PEDRO SATURNINO JR., of legal age, Filipino Citizen, with present
address Brgy. Santiago, General Trias, Cavite, having duly sworn to according to law hereby
depose and state:

1. That I am an owner/holder of a ATM CARD issued by METROBANK;

2. That the said ATM CARD was lost or missing;

3. That I have undertaken earnest and diligent efforts to find for the said lost/missing
in its usual repository and all other possible areas, but to no avail;

4. That I am executing this affidavit to attest to the truth of the statement above and
for whatever legal purpose it may serve.
