A. Specimen received in formalin and labeled with patient's name, MRN and Sigmoid colon
biopsy. It consists of two tan yellow irregular pieces of tissue, measuring 0.3 x 0.3 x 0.2
cm and 0.2 x 0.2 x 0.2 cm. Specimen is entirely submitted in one cassette.

B. Specimen received in formalin and labeled with patient's name, MRN and as Ascending
colon. It consists of two tan yellow irregular pieces of tissue, measuring 0.3 x 0.3 x 0.2 cm
and 0.1 x 0.1 x 0.1 cm. Specimen is entirely submitted in one cassette.
Diagnosis
A. Sigmoid polyp, biopsy:
- Tubular adenoma.
- Negative for high grade dysplasia and malignancy.
B. Ascending colon polyp, biopsy:
- Tubular adenoma.
- Negative for high grade dysplasia and malignancy.
