representative*) hereby undertake to abide with the following below mentioned terms, condition and
undertaking in respect of Provisional clearance of above Livestock(s):

1. I undertake to produce the above livestock(s) along with original health documents at AQCS, New
Delhi immediately after arrival and take provisional clearance. I also undertake that the above mentioned
livestock(s) will be kept under isolation with the required security and biosecurity at above place of
holding without coming in contact with any other livestock.

2. I undertake that officials of Animal Quarantine & Certification Service (AQCS), New Delhi have all
right to inspect the livestock(s) at above holding facility at any time to ensure that the livestock(s)
is kept securely under isolation with proper control and Biosecurity Measures.

3. I undertake that the livestock(s) will not be used for any purpose/s including breeding till final
quarantine clearance is granted by AQCS.

4. I undertake that during the period of holding if the livestock(s) dies or shows symptoms of any
disease, I will immediately inform the AQCS Station and abide by all the instructions/directions issued by
the AQCS in this regard.

5. I undertake to produce the livestock(s) / health & lab report (including PM report if any) as the case
may be after completion of 30 days for obtaining Final Quarantine Clearance Certificate.

6. I undertake and bear full responsibility that in case of non issue of final clearance by AQCS, I will
comply and carry out all directions regarding re-export/deportation/ destruction/ any other issue in
respect of the above livestock(s) as directed by AQCS and other Competent Authority at my own cost.

7. I undertake that the owner will not seek any damages, costs, expenses or other compensations
whatsoever from concerned Officer of AQCS and the Department of Animal Husbandry, Dairying &
Fisheries that may arise due to non issue of final quarantine clearance.

8. I undertake that in case of non compliance of any of the above condition, the owner will not be
eligible to avail the above mentioned facility in future. Besides, the owner will also be liable for any
appropriate action as deemed necessary by competent authority and AQCS.
