Stacking / routine inspection of

quantity and capacity to
cover the hazards
associated with
combustible material at
the work site.

Action required: The


contractor must take
immediate action to
ensure that sufficient
fire points are allocated
for the temporary
storage area. These fire
points should be
unobstructed, clearly
visible, and easily
accessible in case of an
emergency.
Additionally, caution
signage and smoking
prohibition notices
should be prominently
displayed in the area to
raise awareness and
prevent any fire-related
incidents.

Furthermore, the
contractor is advised to
maintain proper
material stacking
segregation to prevent
hazards till final removal
from site. It is essential
to ensure that there are
no obstructions or
obstacles that could
impede the free and
unobstructed access to
walkways. This is
particularly important to
prevent tripping or
slipping hazards.

project and workforce,
which not only violates
our environmental
standards but also
increases the potential
for soil contamination,
and fire hazards. Failure
to address this issue
promptly and effectively
by the contractor may
result in serious
consequences, including
penalties.

Action required:
Conduct an immediate
investigation for the in-
place soil
contamination.
Implement appropriate
containment measures
to prevent further
spread of the oil spillage
and secure the scattered
oil filters, cans, and old
tires. Safely remove and
dispose of the oil
spillage, oil filters, cans,
and old tires in
compliance with local
regulations. Implement
effective housekeeping
practices to ensure the
area remains clean and
free from environmental
hazards.

and safety of our
workforce, which not
only violates our
environmental
standards but also
increases the potential
for soil contamination,
fire hazards, and
workplace accidents.

Failure to address this


issue promptly and
effectively by the
contractor may result in
serious consequences,
including penalties.

Action required:
Conduct an immediate
investigation and
assessment of the area
to accurately determine
the extent of the
hazardous condition
and potential soil
contamination.
Implement appropriate
containment measures
to prevent further
spread of the oil spillage
and secure the scattered
oil filters, cans, and old
tires. Safely remove and
dispose of the oil
spillage, oil filters, cans,
and old tires in
compliance with local
regulations. Implement
effective housekeeping
practices to ensure the
area remains clean and
free from environmental
hazards.

13 January Service Opening / Observation: During a south top area Zone 3 Mazen Rashed Saeed Al-Shehri Active In Progress 24 hours 14/01/2024
Hole Protection routine site inspection
on the south top area, it
was observed that there
are multiple
unprotected service
openings and manholes
along with incomplete
protection on excavated
ditches in close
proximity to major
ongoing work activities.

Action Required:
Immediately barricade
and cordon off the
unprotected service
openings and manholes
to prevent accidental
falls or injuries. Use
adequate covers along
with caution tape and
caution signage to
clearly indicate the
hazards as part of the
control measures.
Ensure that proper
safety covers, lids, or
grates are installed to
prevent accidental falls
or trips.

Regularly inspect and


maintain the covered
service openings and
manholes to ensure
their integrity and
effectiveness.
