HPT Hub Investigation – Background
 Event on engine V12556 occurred 18th September 2014
• During climb, engine experienced a stall followed by high N2 vibration,
EGT over-limit and ENG Fire warning
• Pilot performed a commanded engine shut down and Air Turn Back
(ATB)

 Findings
• HPT stage 2 hub attachment lug fractured and liberated two full blades
which were case contained
• Manufacturing broach process produced non-conforming tool mark
• Inspection did not identify tool mark at one of the suppliers
• Supplier stopped HPT hub production and has not been restarted
