Greetings of Peace!

I am Rohanifah P. Sultan, a newly hired Medical Technologist. I am writing this letter to

inform your office about my concern regarding my Daily Time Record. I was recently diagnosed
with mild hand eczema which affects my fingerprints. Because of this, the biometric scanner is
having difficulty recognizing my fingerprints thus affecting my Daily Time Record. With this, I am
seeking your approval to have my hours of work rendered be recorded manually. I have attached
Medical Certificate and prescription as proof of my condition. Rest assured that the report hours of
work recorded in the DTR form will reflect the amount of time I have rendered.
