HOT WORKS PERMIT
3. Appropriate Fire extinguisher located at the location for each hot work ✔
activity
4. Have all involved personnel undergone proper training / safety briefing
5. Has a documented WAT been completed and all involved personnel aware

of the identified hazards and the required controls
6. Proper housekeeping is done during and after the activity. ✔
9. Welder and fire watcher are complete with the required PPE ✔
10. Warning signs and barriers are in placed (also below if required) ✔
13. Flashback arrestor (for oxy-ace or oxy-LPG) fitted to regulator and torch ✔
14. With proper platform( when activity is in critical area or elevated area)
15. Oxy-Acetylene cylinders are upright and properly secured. ✔
16. All gas cylinders are fitted with working pressure regulator
16. Certified welder (by TESDA or any recognized Agency)

NOTE: This permit must be posted at work area all the time. Erasures, Tampering and/or Unauthorized signatories will be considered INVALID.
