AFFIDAVIT OF EXPLANATION

I, CRISPIN PACATCATIN, of legal age, Filipino, single, citizen and a

resident of Namnama, Jones, Isabela, after having been duly sworn to in
accordance with law hereby depose and say:

1. That I was issued a PhilSys Id from the Municipality of Jones;

2. That on March 08, 2024, when I was on my way to City Hall to pass
the Financial Assistance Documents of our Barangay, I accidentally lost
my said ID;

3. That I tried all my efforts to locate the subject ID but I was not able
to find it;

4. That the said ID is now beyond recovery;

5. That I am executing this affidavit, as required, to enable me to obtain

a replacement for the said lost ID and for whatever lawful purposes
that this statement may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 18 th day of

April, 2024 at Jones, Isabela.
