02. T: Ibigay mong muli ang iyong pangalan at anumang bagay tungkol sa iyong
pagkatao.

S: Opo, ako po si Ma. Luisa Dimayuga y. Dimacali, 20 taong gulang, dalaga,

criminology student at kasalukuyang nakatira sa 279 rivera Caloocan city.

03. T: Anong dahilan bakit nandirito ka sa aming himpilan at nagbibigay ng

salaysay?

S: Andito po ako para magreklamo laban sa taong ito, Abdul Salani, 25yrs old,
single, member ng stribo gang at presently residing sa 9 basag street quiapo
manila.

04. T: Ano ang ginawa ng taong ito laban sayo?

S: Hinold-up po niya ako at kinuha ang bag ko na may lamang pera na umabot
ng 30,000 pesos at cellphone na nagkakahalaga ng 18,000 pesos ( Samsung is
recovered)
05. T: Isalaysay mo ang buong pangyayari?

S: Ganito po yun sir, habang nag-aantay sa kanto ay may lumapit sakin na may
dalang balisong at tinutukan ako sa leeg sa kaliwang bahagi at pagkatapos po ay
kinuha po yung laman ng bag ko at tumakbo palayo at napasigaw ako ng tulong!
Tulong! At may maikling habulan at pagkatapos nun nahuli na po siya kasama
ang balisong.

JOINT AFFIDAVIT OF ARREST

We, PCpl JEREMIE DE JESUS and PCpl CARLO CALLASAN all of legal age
bonafide members of the Philippine National Police and presently assigned as follow up
operatives of station 5, QCPD and after having been duly sworn to in accordance with
the law, do hereby depose and states:

ABDUL SALANI, 20yrs old, male, single, unemployed, violation of article 295 of revised
penal code ( robbery, holdup) on or about 3:00pm September 15, 2023 along maligaya
corner champaca street, Quezon city

THAT, we were conducting repressive patrol when we heard women shouting for help.
instinctively, we immediately chased the suspect that is fleeing. After a short chasing,
suspect were apprehended and recovered the bag of the complainant containing one
cellphone describe a Samsung j1 white color and worth 18,000 pesos and cash
amounting 15,000 pesos. Suspect is appraised his constitutional rights, under R.A7438

THAT, Hereby executing this joint affidavit of arrest to atleast the veracity of the
foregoing facts and circumstances of the arrest of the suspect 15 th day of September,
2023, novaliches, Quezon city.

Ako, MARIA LUISA DIMAYUGA Y. DIMACALI, 20 taong gulang, dalaga, criminology

student, at kasalukuyang nakatira sa 279 champaca st. Quezon city. Pagkatapos na
makanumpa ng naayon sa batas ay nagsasaad ng mga sumusunod:
1. Ako ay dating adik na nahumaling sa illegal drugs ( shabu). Na ako’y nag
sumikap magbago at hindi na muling gagamit ng droga dahil sa nasisira ang
aking pag aaral at ganon din naman ang mga iba pang estudyante na
gumagamit ng nasabing droga. Maraming pamilya ang nasira sa drogang ito.
Kaya’t boluntaryo na akong nakipag tulungan sa hepe ng station 16 upang
mahuli na ang nagbebenta ng illegal drugs at mahinto na ang masamang bisyo
at ang mga krimen. Ang taong nagpapalaganap ng illegal drugs ay si abdul
salani siya ay nakatira sa #9 Champaca street. Quezon city. Siya ay nagbebenta
ng drugs or hayagan na nagbebenta ng drugs sa publiko. Ako at ibang adik ay
pumupunta sa kanilang bahay upang bumili at gumamit. Pag pasok naming sa
bahay may kwarto doon , dun kami gagamit ( kanyang kinukuha sa ibabaw ng
kanyang aparador nang puting box naglalaman ng shabu at paraphernalia. At
pagkatapos naming gumamit at ibabalik na ni (abdul) sa kanyang aparador at
kami ay lalabas na.
Lahat ng sinasabi ko ay pawang katotohanan at nakahanda akong lagdaan ito ngayong
oktubre 13, 2023, siyudad ng Quezon city.

II. This pertains to the above references wherein the undersigned was advised
by PCPT. EDUARDO S. ORTINEZ, chief Investigation section, to explain why
he/she came late gave him 24 hours to explain in writing

On his way to office, the undersigned instinctively responded to a robbery

hold-up incident last November 15, 2023, at 7:00 in the morning. To prove the veracity
of the undersigned allegations, attached herewith are the necessary document such as
affidavit of the arrest, letter to the inquest prosecutor, booking sheet, and arrest report,
medical certificate and the picture of the recovered pieces of evidence. And said case
was assisted by the duty city prosecutor. Thus undersigned had reported late in the
formation.

The applicant PO1 JEREMIE S. DE JESUS of the PS16 Philippine National Police
pasong putik, Quezon city after having been duly sworn, states:

THAT on 18th October 2023, 10:30am personally appeared to the office of Regional
Trial Court branch 11 and reported that the subject of selling and buying of illegal
drugs, ABDUL SALANI located at Champaca 9th street, Maligaya quezon city

(See photos and sketch as annex “A”)

The informer PO1 JEREMIE S. DE JESUS together with undersigned conducted

investigation and surveillance operation at above said address located same address
above. The undersigned together with the deponent inquires to the said office about
application for search warrant. That on the said investigation operation the undersigned
confirmed and believes that abdul salani, and/or any of his accomplice is indeed
engaged in selling and using illegal drugs despite the fact that the said office is illegal
and is not licensed to conduct activities.
The properties, articles objects and items which are used and/or intended to be used in
the commission of the above stated offense in the possession of abdul salani and
includes the following:
A. Methamphetamine Hydrochloride
B. Shabu
C. Abdul salani
D. Box

The undersigned has personally verified the report thru surveillance and investigation
activities to ascertain the veracity thereof and found the same to be true and correct.

PRAYER:
Wherefore, The undersigned Respectfully prays:
A. That the honorable court include in the search warrant and express authority to
conduct the said of the above mentioned premises at anytime of the day or night
including, Saturdays and Sundays considering that these are the days when the
customer traffic are at it’s perk and to break open in the premises therein.

B. That this honorable court causes the immediate issuance of a search warrant
commanding any peace officer to conduct a search warrant on the above
described premises and to seize above described items to be dealt with as the

2. Investigation disclose on or about 2:00pm on October 2, 2023 along maligaya
corner champaca street. Lead by Patrolman De Jesus, Jeremie and
Patrolwomen Mendoza, Maritchell conducting patrol when they heard shouting
for help. Distinctively pointing to suspect immediately cooperative responded and
after a short chasing the suspect apprehended who later identify ABDUL
SALANI, 25 yrs old, Single, Jobless, Residing at Quiapo #9 Basal Manila.
Recover from him the bag of complainant containing one cellphone describe a
Samsung J1 (white) and worth 18,000 and cash amounting 15,000 pesos and
positively identify the complainant as Maria Luisa Y Dimayuga.
