CONTRACTUAL REQUIREMENTS

The Contractor shall refer to the Standard Form of Contract and Technical
Specification for further information. The Contractor may price here for all the
expenses involved, otherwise such expenses are deemed to be included elsewhere.

A Preliminaries and general requirements including provision of working insurances LS


(Workmen Compensation & Public Liability), SOCSO, PERKESO, Work Permit and
other necessary requirement as per FSO's Instruction

B Provide a Performance Bond to Government LS

C Provide a monthly report that records the date, before, during and after the work is LS
completed (hardcopy & softcopy)

D Provide safety equipment and safety signs according to specifications, including LS


repairing any damage to existing roads, temporary paths and safety barrier walls,
storage for equipments, first aid kits, shoes and safety helmet as per FSO's Instruction

STAFFING DURING OPERATION AND MAINTENANCE PERIOD (3 MONTHS)

E Rates for each item in this Schedule of Price are inclusive of Kumpulan Wang
Simpanan Pekerja (KWSP) and Pertubuhan Keselamatan Sosial (PERKESO)
contribution, Uniforms and etc as per Technical Specification for Building Facility
Management & Maintenance

i) Civil Engineer - To be stationed at site Nos 1 3

ii) Facility Management Inspector (Civil Engineer) - To be stationed at JKR's office Nos 1 3

iii) Facility Management Inspector (Quantity Surveyor) - To be stationed at JKR's


office Nos 1 3

iv) Civil/Building Supervisor - To be stationed at site Nos 4 3

v) Site Safety Supervisor -To be stationed at site Nos 1 3

vi) Civil/Building Technician - To be stationed at site Nos 8 3

vii) Incident / Emergency Response Personnel -To be stationed at site Nos 4 3

viii) Site Administration Personnel -To be stationed at JKR's site office Nos 1 3

CONTRACTOR'S SITE OFFICE AND STORAGE

G Provide a facilities for the use of Contractor's staff. The Contractor shall maintain the LS - 3
site office in a thoroughly clean and sanitary condition.

INCIDENT RESPONSE AND DISASTER RECOVERY MANAGEMENT (IRDRM)

H Employ qualified and competent IRDRM team to carry out services including monthly LS - 3
report and equipment as per FSO's instruction and approval.

HEALTH, SAFETY AND ENVIRONMENT (HSE) MANAGEMENT

J To carry out monthly report and services as below: LS - 3

i) Water Quality Monitoring & Report

ii) Noise Risk Assesment Monitoring & Control and Audiometric Test

iii) Air Quality Monitoring & Control

iv) Vibration and groudwater monitoring and control

v) Dust Monitoring and Control

Civil, Structural and Architectural Services

1) The Contractor shall ensure that all groups of assets or systems and
interconnections and ancillary components which may not be referred to
in the Specification in general terms are included for maintenance within
the contract as per Technical Specification for Building Facility
Management & Maintenance
2) Carry out Operation and Maintenance Services for 24 hours including
repairing works and replace consumable part as specified in technical
specification, maintenance frequency schedule and Site Information and
Asset Inventories in Appendix.
