PART A : PRELIMINARIES
CONTRACTUAL REQUIREMENTS
The Contractor shall refer to the Standard Form of Contract and Technical
Specification for further information. The Contractor may price here for all the
expenses involved, otherwise such expenses are deemed to be included elsewhere.
C Provide a monthly report that records the date, before, during and after the work is LS
completed (hardcopy & softcopy)
E Rates for each item in this Schedule of Price are inclusive of Kumpulan Wang
Simpanan Pekerja (KWSP) and Pertubuhan Keselamatan Sosial (PERKESO)
contribution, Uniforms and etc as per Technical Specification for Building Facility
Management & Maintenance

G Provide a facilities for the use of Contractor's staff. The Contractor shall maintain the LS - 3
site office in a thoroughly clean and sanitary condition.
H Employ qualified and competent IRDRM team to carry out services including monthly LS - 3
report and equipment as per FSO's instruction and approval.

1) The Contractor shall ensure that all groups of assets or systems and
interconnections and ancillary components which may not be referred to
in the Specification in general terms are included for maintenance within
the contract as per Technical Specification for Building Facility
Management & Maintenance
2) Carry out Operation and Maintenance Services for 24 hours including
repairing works and replace consumable part as specified in technical
specification, maintenance frequency schedule and Site Information and
Asset Inventories in Appendix.
