JOINT AFFIDAVIT OF DISINTERESTED PERSONS

We, REY STEPHEN CAMAQUIN JR. and NANCY GECANGAO, both of legal age, Filipino
citizens and residents of Brgy. 3, San Pedro, Vintar, Ilocos Norte and Brgy. 4, Sta. Maria,
Vintar, Ilocos Norte, after having been duly sworn to in accordance with law do hereby
depose and state that:

1. We personally and intimately know the late RHANTE DELA CRUZ ACACIO who was
born on July 25, 1966 to Sps. Aser Foronda Acacio and Leticia Cachero Dela Cruz in
Vintar, Ilocos Norte;

2. RHANTE DELA CRUZ ACACIO joined his Creator on April 4, 2022 in Brgy. 4, Sta. Maria,
Vintar, Ilocos Norte, Philippines;

3. Relevantly, however, just recently, we discovered that there are discrepancies in the
personal records of RHANTE DELA CRUZ ACACIO , particularly in the following:

A. NAME OF MEMBER
1. RHANTE DELA CRUZ ACACIO – Death Certificate, Advisory of Marriage,
Marriage Certificate, Birth Certificate of John Raymart, Birth Certificate of
Leticia Mae, Birth Certificate of Joshua
2. RHANTE SR. DELA CRUZ ACACIO – Birth Certificate Rhante Jr.

B. NAME OF FATHER
1. ASER FORONDA ACACIO –Death Certificate, Advisory of Marriage, Marriage
Certificate

C. NAME OF MOTHER
1. LETICIA CACHERO DELA CRUZ – Death Certificate, Marriage Certificate
2. LETICIA CATCHERO DELA CRUZ- Advisory of Marriage

D. NAME OF SPOUSE
1. MARY JEANE VILLAREAL OSTIQUE – Advisory of Marriage, Marriage
Certificate, Birth Certificate of Leticia Mae, Birth Certificate of Joshua
2. MARYJEANE VILLAREAL OSTIQUE – Birth Certificate of John Raymart
3. MARY JEANE V. OSTIQUE- Birth Certificate of Rhante Jr.

E. On the Birth Certificate of John Raymart, the place of marriage of his parents is
incomplete, it should be “San Juan, Metro Manila.

4. Accordingly, RHANTE DELA CRUZ ACACIO and RHANTE SR. DELA CRUZ ACACIO , all
refer to one and the same person;

5. Likewise, LETICIA CACHERO DELA CRUZ and LETICIA CATCHERO DELA CRUZ , all refer
to one and the same person;

6. Moreso, MARY JEANE VILLAREAL OSTIQUE, MARYJEANE VILLAREAL OSTIQUE and

MARY JEANE V. OSTIQUE, all refer to one and the same person;

7. We know the foregoing facts to be true because we are longtime friends and
acquaintances of her Family as we have shared tables during special occasions in our
locality;
 8. The said above-mentioned records are true and to the best of our knowledge;

9. We are also executing this Affidavit to attest to the truthfulness of the foregoing as to
the personal circumstances of the above-mentioned records.
