We will start by doing a fitness self-assessment not so much to score high, but rather to record where
how strong and flexible we are now, so that we can compare how strong and flexible we will be at the end of
this course.

Things you will need:


1. Stopwatch or any watch that measures “seconds”
2. Measuring tape
3. Meter stick/Ruler
4. Weighing scale

Record your performance in the tracking sheet found on the last pages of this Student Activity Sheet
(SAS).

III. Activity 3: Fitness Assessment Form (10 pts.)


Teaching-Learning Activities:
1. Prepare the tools for testing body composition, anthropometry, speed, agility, flexibility, strength, and
aerobic and anaerobic capacities. Show pictures of all the tools you created in the Documentation
Part.
2. Measure your height and weight and compute your body mass index.
3. Administer the tests appropriately and accurately to yourself. Prepare documentation (picture) of all
the tests you performed.
4. Accomplish completely the fitness assessment form shown below before performing any Physical
Fitness Test.
5. Evaluate and interpret the results.

IV. Activity 4: Return Demonstration (50 pts.)


Direction: In continuation of your Module 2, kindly perform all the tests below. You must put the data on the
given tables. Do not forget to take documentation (Pictures ONLY) for each test.
Note:
a. Who will not join the Fun Run
In Module 2, you have done performing the IE (Initial Evaluation) of each test, for this particular module, you
need to include all your data from Module 2 and perform the 1st RE (Re-Evaluation) ONLY.
b. Who joined the Fun Run / Community Zumba
For those who will join Fun Run, you will not perform the Return Demo but instead, we’re going to have a
F2F Community Zumba Pre-Performance for the upcoming FDC. Practice the Zumba steps through this link
(https://drive.google.com/file/d/1Fpq_9JVSZ3I5-tCBk-MP-VAmHkJNdFwG/view?usp=sharing), and the
schedule for F2F Pre-Performance will be posted on our FB Group. Ensure to keep documentation during
