I, Professor Dr. M.A Samad, am writing this letter to confinn the medical condition and
dependency status of my patient, Mrs. Razia Alam (DOB: 10 Nov-1957, National JD:
5057737594). Mrs. Alam has been under my care for the past 02 years. Patient ID: 2023045945.

Mrs. Alam suffers from multiple chronic conditions including Parkinson's disease, a history of
brain stroke, stage- 4 chronic kidney disease,. and advanced arthritis. These conditions severely
limit her ability to perform daily activities independently. In addition to her physical ailments,
Mrs. Alam ha. developed symptoms of depression and anxiety, further impacting her overall
well-being.

Over the past six months, Mrs. Alam's condition has deteriorated significantly. Her mobility has
drastically decreased, necessitating assistance for even basic movements. She now requires
constant help with personal hygiene, medication management, meal preparation, and all
household tasks. Her cognitive function has also declined, affecting her ability to make decisions
and manage her affairs. Her mental health concerns, coupled with her physical limitations, make
it crucial that she receives dedicated, continuous care in a supportive environment.

., In my professional opinion, Mrs. Alam's level of dependency is severe and will continue to
increase. Specialized, full-time care is essential for maintaining her health, safety, and quality of
life.
