1. Abstract

Introduction: Excessive gingival display is a cosmetic problem where


it is usually managed by different approaches such as lip repositioning .
In this study, we will evaluate the modified lip repositioning surgery and
z plasty frenectomy as technique for treatment of gummy smile patients.
Aim: the aim is to evaluate the modified lip repositioning surgery to
reduce the gingival display and increase patient satisfaction by a simple
surgical procedure .

2. Introduction and Background
Excessive gingival display (EGD), commonly termed gummy smile, is a
condition characterized by an overexposure of the maxillary gingiva while
smiling .It is distinguished by showing more than 1.5-2 mm of the gingiva
.The amount of discrepancy considered unattractive varies between
populations; however, an excess of more than 3 mm is agreed upon
worldwide

Presented a case report of 25 year old patient with excessive gingival display
using lip repositioning surgery where the technique involves the removal of a
strap of epithelium from the vestibular mucosa thereby shortening the
vestibule, the proposed technique can be used an alternative to invasive
surgery with satisfactory results. It can be used alone or in combination with
other procedures , the advantage of this technique is that it is an easier less
complicated approach with low morbidity providing a satisfactory
camouflage effect (Foudah MA 2019)

Botulinum toxin injections and lip repositioning surgery are the primary
options for the treatment of excessive gingival display; orthognathic
surgery is another option. However, botulinum toxin injections do not
have a permanent effect and must be repeated every few months,
whereas orthognathic surgery is an invasive procedure (Polo,

accurate case selection is the key in preventing the relapse associated with the
procedure where if followed correctly the prognosis of the technique ill be

All patients will be given antibiotic drug (Augmentin 1gm tab twice daily
for one week), anti-inflammatory drug (alphintern 1 tab three times per day
for one week) and analgesic drug (Cataflam 50 mg one tab)

‫بعد التشخيص الدقيق ويعتبر المريض مؤه ًل سيتم اجراء العملية الجراحية و متابعة النتيجة من‬
‫حيث التئام الجرح و الظهور اللثوي ورضا المريض‪.‬‬

‫تعتبر االبتسامة اللثوية من المشاكل التجميلية التي تنتشر بشكل شائع لدى كثير من الناس‬
‫وباألخص السيدات يمكن عالج هذه المشكلة بعدة طرق مثل حقن البوتوكس في جزء من‬
‫عضالت حول الفم والشفة العلوية‪ ،‬ومن الممكن عالج االبتسامة اللثوية من خالل حقن الفيلر لكن‬
‫هذه الطرق تعتبر مؤقتة‬

‫اما عملية اعادة تموضع الشفة تتميز انها ال تحتاج الى تخدير كلي بجانب قلة المضاعفات‬
‫المصاحبة للجراحة وسهولتها بالمقارنة مع جراحات اخرى‪.‬‬
