AFFIDAVIT OF LOSS

I, LUISITO MILITANTE RANZE, of legal age, married, Filipino, and a resident

of Block 21 Lot 24 Phase 2B V&G Subdivision Tacloban City, after having been duly
sworn to in accordance with law, do hereby depose and state that:

1. I currently have an account with the Bank of the Philippine Islands (BPI) with
number 3529182642 for which I was issued a corresponding ATM Card with
number 536347 0017 2472 35 under my name by said bank;

2. I kept said ATM Card in my wallet at all times for ease of access whenever I have
to use it;

3. Sometime last month, I was about to use said card to withdraw when I
discovered that the abovementioned card was missing;

4. Upon realizing such fact, I took all the pains in trying to find said card but all of
my efforts in the end turned to be futile;

5. On account of time constraints, I hereby consider my card as having been totally

lost beyond recovery for all legal purposes; and

6. I am executing this Affidavit to attest to the truth of the foregoing and to apprise
the proper authorities for the issuance of a new one and for other purposes.
