I, MA NICACHIE R. TOR, of legal age, Filipino, married, and a resident of

Marasbaras, Barangay 80, Tacloban City after having been duly sworn to in accordance
with the law, hereby depose and state that:

1.) I am the mother and legal guardian of DHEMPSEY SOPHIA R. TOR, who is
presently enrolled as a student in the Senior High School curriculum at Holy
Infant College;

2.) Currently, I am not employed and do not have any ongoing business or
professional activities; and

3.) I am executing this Affidavit to attest to the truth of the foregoing and in support
of the application for scholarship of my daughter with the Holy Infant College.
