3 DUTIES AND RESPONSIBILITIES
A) PROJECT CO-ORDINATOR (AT HEAD OFFICE) -

1. Initial start-up process at site including manpower planning.

2. To monitor for timely deployment of required man power resources at project site.

3. To monitor Bar Chart, Plan/Time of execution.

4. To ensure all safety compliances at project site.

5. To finalise engagement of Sub -Contractors in coordination with Departmental Heads/Higher


Authorities.

6. To ensure that all records including correspondences are properly maintained with the client.

7. To liaise with Government/Local bodies, Clients, for project related works.

8. To monitor project review and update the Management of the monthly physical program status.

9. To coordinate with other relevant departments at Head Office, viz., Finance, Materials, HR
and other concerned departments in relation to project related works.

B) SITE IN-CHARGE

1. He shall shoulder responsibilities for the prevention of accident in the project site and
implementation of Health and Safety policies at site.

2. He shall co-ordinate with the client in regard to the requirements of procedures for safety and
certify areas/equipment’s, etc. for safe working after thorough investigation, as necessitated by
the Client..

3. He shall enforce all General and Safety Rules and Regulations.

4. He shall be familiar with statutory legislations and client safety guidelines.

5. He shall constitute a Safety Committee at site and conduct periodic safety meetings.

6. He shall organise Training Programmes and ensure participation of his workers for training as per
schedule arranged by Safety Department and the client as well. .

7. He shall inspect work areas regularly with members of the Safety Committee.

8. He shallt ensure that permit system is followed properly at site.

9. He shall ensure that Personal Protective Equipment (PPE) supplied by the Company are used by the
personnel at site invariability, depending upon the nature of job entrusted.

10. He shall impart training to site personnel about the importance of Health and Safety

11. He shall provide the Safety Department at Head Office with necessary information
required to evaluate the effectiveness of the Health and Safety Programmes.

C) SITE SAFETY HEAD/SITE SAFETY OFFICER:-

1. He shall be responsible for all the safety activities and programmes at site.

2. He shall conduct Induction Training Programme for new employees before they
are being permitted to start work at site. Also, he shall provide necessary training
for new employees in relation to safe working methods, hazards and risks related
with the job, use of Personal Protective Equipment, House Keeping, etc..

3. He shall inspect the work stations at site regularly and unsafe acts or unsafe
conditions, if any observed, will be reported to the Site In-Charge and the Head
Office invariably.

4. He shall organize monthly safety meetings, duly co-ordinating with Site In-Charge
and the MOM recorded thereof shall be sent to all concerned invariably.

5. He shall investigate all accidents, etc. and prepare a report thereof. He shall also
suggest corrective action plan to avoid such accidents in future. All such reports
shall be sent to Safety Department at Head Office periodically.

6. He shall ensure that all records pertaining to site safety are properly maintained
and made available on demand by the authorities concerned.

7. During his routine visit of the work areas/site premises, if there are any unsafe acts
or conditions observed, he shall immediately stop the work and bring it to the
notice of the Site In-Charge, Site Engineer or the Site Supervisor. The work shall
be resumed only after attending to repairs, etc. and after making the work place
safe.

8. He shall periodically check that sufficient Personal Protective Equipment are


available at site.

9. He shall check the necessary factors before allowing/sanctioning any Work Permit
(s).

10. He shall check all equipments, tools, tackles and other accessories, which are
used at site and ensure that they are in good working /usable conditions.

11. He shall motivate continuously all personnel at site regarding effectiveness of


Safety.

D : SAFETY STEWARDS :

1. He shall exercise close supervision over employees, who are engaged at work.

2. He shall take immediate corrective action whenever/wherever unsafe


act/condition/practices are discovered.

3. He shall ensure that all equipments are properly handled and used during the
job.

4. He shall be able to respond to all emergencies including Fire incidents at site.

5. He shall be a holder of Certificate in First Aid and provide First Aid in an


emergency on a basic level.

4. HEALTH AND SAFETY GUIDELINES
1) No workmen below 18 years of Age shall be engaged for a job.

2) All workmen shall be screened before engaging them on the job. Physical Fitness Certificates of
the persons to certain critical jobs like working at height, working in confined space ,entry etc. and
other dangerous locations are obtaining before engaging such persons on work at site.

3) Smoking is strictly prohibited at work place and also working under the influence of alcohol or
any type of intoxicating drugs is completely banned.

4) Before commencement of any work by the site personnel, they shall understand completely the
procedure, etc.. While executing the assigned work, if any difficulty or problem being faced by
the workmen, he shall immediately contact his superiors for resolving.

5) Sub - contractors shall ensure adequate supervision at work place. They shall ensure persons
working under them shall not create any hazard to self or co-workers.

6) Good House Keeping shall be maintained in all work areas.. Passages shall not be blocked
with materials. Materials shall not be stacked on the landing areas of the staircase. Also,
ensure that there are no criss-cross wirings on the floors.

7) Horse play is completely prohibited at workplace. Running at the site is completely


prohibited, except in case of emergency.

8) Ensure adequate First Aid boxes with Medicines are available in the work stations and shall be
accessible to everyone. Also, ensure that the medicines in the First Aid Boxes are sufficient for
emergency use as per the work activity.
9) At project site, usage of Safety Helmets, Safety Shoes and other PPEs are compulsory.
Nobody is allowed at site without safety helmets, safety shoes and other PPEs provided
by the Company.

10) Sub-Contractors should arrange safety shoes and safety helmets and other necessary personal
protective equipments (PPE’s) as per requirement of any job. Sub- contractors are responsible
for providing necessary PPEs to their workers.

11) Usage of eye protection equipment shall be ensured when workmen are engaged for
grinding, chipping, welding and gas cutting. For other jobs as and when HSE officer insists
eye protection has to be provided.

12) All PPEs like Safety Helmet, Safety Belt etc. shall be arranged before starting the job as per
recommendation of HSE officer and ensure they are in good conditions.

13) Adequate illuminations at workplace shall be ensured before starting the job at night. If
illumination is not sufficient, necessary provision of illumination should be made.

14) Ear plugs or Mufflers may be used for ear protection where there is high noise.

16) All the dangerous moving parts of the portable / fixed machinery being used, shall
be adequately guarded.
17) Proper access and working platforms shall be provided to reach the work spot.

18) Ladders being used at site shall be rigid in condition and shall be adequately secured
at bottom and top. Ladder shall not be used as work platforms.
19) All scaffoldings / work platforms shall be strong enough to take the expected load. The
width of the working platform and fall protection arrangements shall be maintained as
per recommendation of safety officer and project dept. Approach ladder (MS or
Aluminium) shall provide for the entire working platform.
20) Materials shall not be thrown from the height and shall only be lowered through proper
arrangements. If required the areas shall be barricaded and one person shall be posted
outside the barricading for preventing the trespassers from entering the area.
21) Electrician is only the authorised person, who is allowed to carry out electrical
connections, repairing or any work related with electricity. No any other person is
allowed to do such work.
22) Power supply shall be taken through Earth Leakage Circuit Breakers (ELCB) only.

23) Inserting of bare wire for tapping the power from electrical socket is completely
prohibited and Metal Clad Plug Tops must be used. Also the condition of insulation for
cable/wire must be checked before use.
24) All Tools and Tackles shall be inspected before use. Defects, if any, observed shall be
reported immediately. If the tools and tackles are not in good working condition, shall
have to l get replaced before use.

25) Debris, scrap and other materials shall be cleared from time to time from the workplace.

26) While doing welding work at height or on floor, a sheet / fire blanket shall be placed
below the work area to prevent welding spotters falling from height.

27) Employees deployed for height work(s) shall be selected persons, who are
experienced, possessing adequate knowledge and screened and inducted by the site
management.

28) Workmen shall wear full body harness safety belt with double lanyard for works at
height. The employee shall hook his safety belt once he reaches the work spot.

29) Welding cable shall be kept tied in Gratings for fall protection.

30) Connection of the welding cable with the output pole of the welding machine shall be
suitably done with proper fittings such as legs. Welding joints shall be made using
legs and the joint shall be well insulated.

31) Electrode holder shall be of good quality with proper insulation .The welder should never
be allowed to weld when he is wet. Usage of proper personal protective equipments
shall be ensured.

32) During rainy season, welding work shall not be carried out in an open atmosphere.
There shall be sufficient protection from rain.

33) Combustible and inflammable materials shall be removed from the place where welding
activity is carried out.

34) Welding Cable shall be routed through ELCB and kept tied in Gratings for fall
protection.
35) Welding cables shall not be laid along with other electrical cables, as they may
damage insulation of other cables.

36) In use of Gas Cutting set, a flashback arrester (at regulator and cutting torch end) shall
be used.

37) Barricading wherever necessary may be provided at workplaces.

38) Standing under the load lifting area by cranes, hydras are prohibited.

39) While doing any hot work, a fire extinguisher will be kept at side.

40) Housekeeping will be carried out as per the schedule.

5 Personal Protective Equipments
Personal Protective Equipments (PPE) –
Following measures shall be followed at site.

Promac Engineering Industries Limited shall provide all required Personal


Protective Equipments to all employees. They include safety helmets, safety
shoes, eye/ face protection, gloves, full body double lanyard safety harness and
hearing protection.

All employees of Promac Engineering Industries Limited shall wear safety helmets and
safety shoes at project site. Nobody shall be allowed to enter the site without wearing
these minimum two personal protective equipments.

All Personal Protective Equipment shall be of good quality.

For employees, necessary training shall be provided regarding proper usage of


personal protective equipment.

Besides employees at Site, the Office employees and visitors of Promac Engineering
Industries Limited all always wear safety helmets and safety shoes.

As per the safety norms and requirement, necessary Personal Protective Equipment
shall be used for various operations.

For electricians, rubber gloves shall be provided.

Hearing protection shall be used, if required.

Above 1.8 meters height, positive fall protection (double lanyard body harness, fall
arrester etc.) shall be used. If fall protection system cannot be provided, then
alternative working method shall be developed.

If necessary, fluorescent jackets shall be used at site as per requirement.

Following Matrix for Personal Protective Equipment shall be observed at site.

6 Hand and Portable Tools
Most hand tools like wrenches, hammers, electrically powered hand tools needs
inspection before use. As per list given below, necessary precautions shall be taken.

Before use, every hand tool shall be inspected and ensure that the tool is in
good working condition.

If any tool or equipment does not withstand in inspection, then the same shall
be got repaired or replaced.

For hand power tools, checking shall be done for condition of guards for moving
parts. They shall be used only for work for which they have been designed.

To be operated only by personnel who have been authorized and given appropriate
training.

Head of hammers and other shock tools shall get dressed or ground to a
suitable radius on the edge as soon as they begin to mushroom or crack.

Pneumatic tools shall be operated by trained persons only.

Hose and hose connections shall be designed and tested for the pressure and
service for which they are intended.

Pneumatic tools shall be disconnected from power and the pressure in hose lines
released before any adjustment or repair is made.

Electric power operated tools used shall be of good and approved insulated type
and with earthing provision.

7 Scaffolding and Portable Ladders
For scaffolds, following precautions shall be observed at site.

Only metal scaffolds shall be used at site.

Construction of scaffold shall be done by experts and senior persons.

Only after inspection, the scaffolds shall be used for work. Also safety head/officer
and Project Dept. shall inspect the same.

Scaffold shall be of good construction, suitable and sound material and having adequate
strength for the purpose for which it shall be used.

The scaffolds shall be secured by both ties and braces to prevent movements.

Base plate, sole plate, planks, top rail & mid rail, etc. shall be provided for scaffold.

Working platform with handrail approximately one meter height, mid rails and toe
Boards etc. shall be provided for scaffold.

To allow access to the working platform a stable stairway with railing shall be
provided for scaffold.

Tags such as ‘Out of Service’ ‘Ready for Use’ ‘Under Construction’ etc. shall be
provided on scaffolds.

Before erecting scaffolds, all nearby and overhead energy hazard sources e.g.
electrical, mechanical, pneumatic shall be considered and erected in safe condition.

Portable Ladders-

All ladders shall be checked and inspected prior to use by Safety Head/Officers

Metal ladders shall not be used for any electrical repairing, electric welding or near
charged electric lines.

Defective ladders shall be replaced if any defect(s) is/are noticed..

8 Lifting and Shifting Equipments
Following are the general guidelines shall be followed with all lifting and shifting tools,
tackles and equipment.

All lifting appliances shall be examined by competent persons (third party inspection) at
frequencies as specified in ‘The Factories Act’ and the test certificates shall be maintained.

Quality, size and condition of all lifting tools like wire rope, chain pulley blocks, slings, D-
shackles and jack etc shall be checked thoroughly before putting them in use.

Before making the lift, make sure the weight and balance of the load are known and the
sling is securely positioned around the load.

9 Cranes, Hydras and Winch Machines

Crane and hydra shall be examined by competent persons (third party inspection) at
frequencies as specified in ‘The Factories Act’ and the test certificates shall be
maintained.

Insurance and registration for crane shall be ensured and the records / proofs shall be
maintained.

Only qualified operator, having proper license shall operate crane.

Load limit device shall be provided on hoist.

Training programmes for crane operators signal men etc. shall be arranged. It
mainly includes safe lifting techniques, use and types of ropes, slings.

Also training program shall include lifting and shifting material by hydras/ cranes.

Only qualified person shall operate hoist. and certification of operator shall be submitted to
Client Project or safety department before starting the job.

Lifting equipments like cranes, hydra which shall be used at site shall have valid documents
like insurance, test certificates etc. obtained from competent person.

The SWL, date of testing, due date of testing shall be clearly marked on the body of
crane.

Winch machines shall be examined by competent persons (third party inspection) at
frequencies as specified in ‘The Factories Act’ and the test certificate will be
maintained.

For winch machine installation and operation process, all precautions and checklist shall
be observed.

Safety head or safety officer shall be supervising all the checking/inspection of winch
machines.

Condition of rope shall be inspected for damage, if any.. If rope is damaged, it shall
be replaced.

Winch machines shall be installed at locations where the Concrete column or steel
column is available.

Ground floor area shall be barricaded or protected.

Necessary records shall be maintained in specific format or in logbook.

Damaged winches shall not be used, repairing or replacement shall be


carried out..

10 House keeping

Every person is responsible for a good and proper housekeeping.

Housekeeping for immediate work area shall be carried out to ensure safety and
efficiency. Storage area shall be clearly identified and barricading provision done.

Adequate lighting and walkways for safe access, labelling for items, shall be provided at
storage area.

Empty drums or containers with labels shall be provided at specific locations at site
for collection of waste/ scrap or unwanted material.

It shall be ensured that the drums or containers will get empty after regular interval to avoid
overfilling.
Material spills at yards, grounds and roadways shall be cleaned immediately.

For housekeeping awareness, safety and warning signs, boards shall be displayed on
various locations on site.

Obsolete equipments or materials shall not be kept on access, walkways and roadways.

Safety Head/Officer shall make round on site daily and supervise total

housekeeping. Bulletin boards, notice boards shall be kept properly, updated and

neatly. Street signs and road markings shall be kept in good condition.

11 Working at Height

During work at Height job, necessary precautions shall be taken. Our safety

Head/Officer shall supervise continuously height work job.

Any work which is above 1.8 meters shall be considered as work on height.

As per work requirement platform, ladders, scaffolds etc. shall be provided.

To ensure that only experienced person shall work on height work job.

Use of Personal Protective Equipment, viz., Safety Helmets, safety shoes, safety
goggles, full body safety harness, etc . shall be used.

Proper training shall be provided for persons and they are informed about risk and
hazards involved in the job.

For working at height fall arrester shall be provided and to prevent falling objects
from height, provision of safety net shall be done.

Life line shall be provided wherever required.

Tools and tackles which are required shall be kept properly tied during height
work. Sufficient illumination shall be arranged during height work job.

Barricading shall be provided at bottom area of height work.

To maintain calm and cool at height work.

To avoid over exertion.

Work at height jobs shall be supervised by the Site Safety Head/Officer.

12 Working in Confined Space

A Safe Work Entry Procedure (SWP) shall be prepared and then permit shall
be obtained.

Training shall be provided to persons who are involved in job (persons making entry
in confined space, persons supervising and stand by person.)

To ensure proper and accessible means of exit before entering into the confined
space.
To ensure proper ventilation and exhaust fan arranged for confined space.

Only 24 volts electric lamp shall be allowed and used in confined space work. No
other type/ ratings of lamp shall be permitted.

Oxygen contents shall be checked and monitored in confined space.

Persons who are entering in confined space shall wear safety helmets, Safety
shoes, safety belts and other necessary personal protective equipments.
Presence of toxic/flammable gasses shall be checked.

Compressed gas cylinders will be kept outside of confined space.

When the work is going on in the confined space, there shall always be at least one-
person stand by at outside.
At the end of the work, ensure that all persons, materials, tools and
equipments etc are removed from confined space.
After completion of work, ensure that all persons are coming out of the
confined space and the permit will be cleared.
Supervisor and safety officer shall be given responsibility to ensure that
necessary precautions are taken.
Persons working in such places shall have sufficient experience and training.

13 Excavation
Proper safety procedures shall be prepared and clearance be obtained considering the
underground above ground utilities.

A permit shall be prepared before starting actual excavation work.

Condition of soil is considered and as per soil condition provision shall be made for stability
given to excavated pits.

Training shall be provided for persons doing excavation job and they shall be informed
about hazard and risk related with excavation.

Proper and strong barricades shall be provided and warning flags, boards, red night
lamps shall be arranged.

Proper Access and Egress shall be provided for every Excavation.

14 Protections from Openings at Floors
Any openings at floor or opening protection for floors, walls, and roof edges have high
potential for serious injury. Proper and adequate protection is required to prevent
personnel or material from falling through openings at floors or walls and from the edges of
roofs or floors.

All openings or edges that could expose personnel or material to a fall hazard shall
be properly hard barricaded or protected.

Such openings in floors, walls etc. shall be covered or closed with the grating, chequer
plate, MS plate or by suitable source.

For proper locking of plate or grating on openings, clits shall be provided from inside or at
bottom of plate or grating so that proper and safe locking will be done.

The personnel who are working within 2 meters area of an unguarded opening or edge
shall be used full body safety harnesses with lifeline. Also the below area shall be
barricaded and warning signs provided.

Warning signs shall be provided for covers e.g. ‘Danger’ ‘Hole Cover’ ‘Do Not
Remove’.

Any materials or equipment shall not be stored on hole covers.

Ladder way floor openings or platforms shall be guarded by standard railings with
standard toe boards on all exposed sides, except at the entrance to the opening.

15 Welding and Gas Cutting
Welding-

Electric welding is very important operation in any mechanical fabrication process. It


also involves various major Hazards and risks. Following precautions shall be
observed in welding operation.

First of all necessary information shall bel given to concerned persons regarding
hazards and risks involved in welding.

AC transformers shall not be used, only AC rectifier shall be used.

Before connection, the Electrician shall check every rectifier for its condition. If the
same is not in operating condition, tag will be displayed on it.

Only the Electrician is authorised for connection.

For welding machine supply should be given through Earth Leakage Circuit Breaker
(ELCB) having ratings 30ma.

Ensure conditions of supply cable and they are suitable for current required by
machine.

Welding leads should be checked before starting work for its insulation. Also for
connection of leads, cables plug will be used.

Provision of Earthing shall be direct the job or object at which welding being in
progress. Electrode holder must be getting checked for its insulation.

There shall be no joint for cable connection for welding machine and electrode holder.

Welder shall use the personal protective equipments i.e. Safety helmet, shoes, apron,
welding screen, gloves etc.

After the work or when machine is not in work, always keep machine in ‘OFF’ mode.

Near welding place a suitable fire extinguishers shall be arranged.

Gas Cutting-

Following Safety will be ensured for Gas Cylinders while transporting, handling and
at time of using.

Cylinders shall be transported with their cap ON and through proper trucks or trolleys.

Cylinders are moved properly by means of suitable trolleys having locking


arrangement.

While using the Cylinders regulator, pressure gauge, flash back arrester (non- return
valve) shall be used.

Acetylene cylinder key for opening valve shall be kept on valve stem. While cylinder is
in use, so that the acetylene cylinder could be quickly turned off in case of emergency.

Check valves and Flame arresters shall be used for Compressed Cylinders.

Colour code shall be followed for hose pipe. (Blue hose pipe for Oxygen and Red
hose pipe for D.A.)

Condition of both hose shall be checked.(for leakage or damage)

Full compressed and empty cylinders shall be kept always in vertical position with
chain or cable fastened.

Compressed Cylinders shall not be taken into confined spaces.

Cylinders shall be stored in areas where they will be protected from falling
objects, source of heat and electrical wiring.

Full and empty gas cylinders shall be stored separately.

From monsoon and sun heat protection, shed shall be arranged for
compressed cylinders.

For all components of oxy-fuel equipment, there shall be a complete inspection
at regular intervals.

Portable fire extinguishers shall be arranged near oxy-fuel work places.

Lighted gas torch shall never be left unattended.

Only qualified person shall operate the oxy-fuel system with having necessary
personal protective equipments.

Proper training shall be provided for concern persons at regular intervals.

16 Radiography

For Radiography purpose, radioactive material source used shall be as per


the guidelines.

Before radiography, plan of storage, transportation and handling of source shall


be submitted to Client.

Before installing the source, entire effective area, where the N.D.T. is to be carried out,
shall be properly barricaded. Warning signs, boards are posted and red lamps
provided around the area.

Our safety head/officer shall ensure the above precautions before obtaining the
permit from Client.

The contractors and agencies which are working nearby areas of radiography shall
be informed well in advance.

As for as possible, field radiography shall be done only during night time. Under
unavoidable circumstances, it shall be possible during day hour at between lunch
hours, for those adequate safety arrangements shall be made.

Entry into the restricted area by unauthorised persons shall be strictly prohibited
during exposure. Our Safety Officer shall ensure this.

The radioactive source shall never be touched or handled directly with hands.

All personnel working with radiography sources shall wear appropriate protective
equipments.

Radioactive source shall be stored at isolate area where minimum public movement
is considered. Warning symbols shall be arranged for this storage area.

17 Work Permits System

Work Permit system shall be followed strictly on site. It shall ensure that every work

shall be started only after receiving particular type of permit.

Regarding every work, responsibilities shall be fixed, for all persons involved in a
particular work.

For each permit, its boundary limitations and time limit shall be specific mentioned on
permit.

As per requirement of work, permit shall be extended as per Client’s rule and
procedure.

All necessary precautions and safety measures shall be followed as stated in work
permit.

As per work requirement, permit shall be issued from Client.

Training shall be provided for better improvement of work permit system.

18 Electrical Safety Guidelines
Following electrical safety measures shall be observed.

Electrical Rule rules and regulations shall be followed.

Qualified Electrician shall be appointed on site and only he shall be the authorised
person for any electrical work, connection etc.

Only industrial type plug and sockets shall be used.

For every installation, proper earthing shall be provided.

As per requirement, Electrical distribution boxes shall be provided on various locations


on site.

Rain protection shall be provided for electrical installations and distributions boards.
Joints for cable shall be minimum and cable jointers shall be used for cable joints.
Only three-core double insulated cable shall be used.

All the wiring shall be overhead (above ground at least 2 meters height) or wiring shall
be underground.

30 mA or less rated ELCB’s will be used on electrical installations, for welding


machines and portable hand tools.

Fuse shall be provided for electrical circuits or equipments at necessary locations.

It shall be ensured that the handles, body of portable electrical tools shall be of rubber or
plastic and in good conditions.

For work in confined space entry, only 24 volts lamp shall be used.

Before giving connection or charging for any machine or tool, charging or electrical
permit will be obtained.

Proper PPE’s like electrical safety shoes, rubber hand gloves shall be used during
electrical work.

Suitable electrical warning boards will be displayed on site like ‘Danger’ ‘Stay away’
‘Do not touch’ etc. Board shall be in Hindi or local language.

For live electrical cables, any type of work shall never be carried

out. Naked wires shall not be used.

MCB shall be used in circuits.

All illumination lamps shall be protected from accidental contact and breakage.

19 Lock Out / Tag Out System
For electrical safety purpose, lock out and tag out procedure shall be required and

followed on site.

Only electrician is authorized and permitted to do lockout / tag out procedure.

Engineers, supervisors, electrician and safety officer shall get involved in this procedure
as per requirement.

As per work requirement, necessary lock out and tag out plan shall be prepared.

Permit shall be prepared first for lock out/tag out.

Suitable tags shall be arranged for lock out/ tag out.

Safety officer shall observe and ensured that lock out / tag out procedure is properly
done.

Persons shall not operate devices, which have lock out, or danger tags.

About lock out and tag out system, training shall be arranged for all persons.

20 Access and Visitor Control Security Procedure
Material or equipment which shall be moving in and out shall be accompanied by
approved gate passes.

All personnel who bring their property like personal computer, tools, equipment etc. shall
declare at the security gate/check point and obtain material gate passes.

If there is any work planed on Sunday or any declared holiday, formal permission
shall be obtained from concerned authority of client. Adequate supervision shall be
provided.

All motor vehicles, which are used at site, will have a valid license issued by Regional
Transport Office (RTO) and insurance coverage, etc..

Condition of the vehicle shall be checked and ensure that its tyre, engine, working front
and backlights, horn and reverse horn, mirrors are in good condition.

Project personnel shall be prohibited from ride on loads, sideboards, tailgates etc of
trucks. Transportation of persons shall be done through passenger vehicles only.

Persons shall not allow riding on any mobile equipment where proper seating is not

provided. Mobile equipment shall be taken reverse only with the assistance of a signalman.

21 Education, Training and Safety Awareness

All employees and sub-contractors’ employees (if any) of M/s.Promac Engineering


Industries Limited shall attend the project site safety orientation/induction training.

Tool box talk / pep talk shall be conducted on daily basis at site. All workers and
staff members shall attend the same. Without tool box talk work shall not be
allowed to start.
Tool Box Talk -

Every day morning, before starting the job a tool box meeting or tool box chat shall be
held by site safety Head or officer at site. All staff members and workers shall attend
the tool box talk.

Job Safety Analysis (JSA) –

M/s.Promac Engineering Industries Limited will carry out the required Job Safety Analysis.

Mainly Job Safety Analysis shall be carried out where the task shows a significant
hazard or where there may be hidden hazards or where high degrees of co-
ordination between workers are required.

22 First Aid / Medical Facilities

We provide highest priority on the safety and health of all personnel working on the project
site.
Number of severity and occupational injuries may be reduced by effective prevention.

If any unplanned events occur, it is important to have adequate emergency medical services
to help the situation so that prompt and competent medical care will be provided.
1) M/s. Promac Engineering Industries Limited will maintain the sufficient first aid
boxes at project office and at necessary locations on site.

The first Aid box shall be protected from the dampness and dust.

The contents of first aid box shall be inspected by safety head/officer at regular interval.

In case of emergency, medical assistance / help shall be obtained from


designated hospital and physicians who are having relationship with client.
2) Transportation -

M/s. Promac Engineering Industries Limited will provide a suitable vehicle for
employees /staffs to transport them to the Medical Facility during any emergency.

Also transportation shall be provided for all follow up visits to medical facility related
with any person.

3) Medical /First Aid Treatment –

If there is any injury for employee at project site, and requires treatment beyond the
limit of the medical attendant, Promac shall transport the injured person to hospital.
4) Injury Documentation –

An injured employee shall report all occupational injuries to his foreman or


supervisor before he seeks any first aid facility on that day or onset of illness.

Records of first aid report shall be maintained to check how the injury occurred
and what medical care is provided. It also gives information as to whether the said
employee has been referred to a physician.

This daily records will consist of name of employee, employee number, date and
time of injury/illness, type of injury, nature of work being performed, his supervisor
and treatment given etc.

23 Accident / Incident Investigates and Analysis
All incidents, regardless of severity shall be reported.

Incident investigation team shall conduct a written investigation. This incident includes
near misses, illness, equipment damage etc.
All reports and investigations shall be maintained and Corrective actions taken shall
be properly documented.
A) Incident Reporting-

Employees shall report any unsafe act or condition to their supervisor or Safety Officer or
Safety Head. Safety Head/Officer shall provide training to them.

When an accident or near miss occurs and even if there are no injuries to employees,
the root cause of the accident shall be investigated. Any unsafe or inadequate policy,
procedures shall also be reviewed and necessary corrective action be taken.

All near misses, incidents / accidents shall be reported, documented & records shall be
kept properly.

Incident or accident report shall be very clear and in details about what happened, to whom,
when, where, nature of injuries, damages and losses that occurred. All necessary
information shall be provided.

Reporting of near miss accidents shall be strictly done.

Classifications of injuries and accidents shall be carried out if required.

B) Near Miss Accident – An accident or a situation or an event having potential to cause


injury, damage to property, environmental release or an adverse community reaction
is known as near miss accident.

Even though no actual injury, damage for property or any negative consequences
happened, the study of near miss incident is very important from the angle of accident
prevention and safety.

Doing the study of near miss accident, we can analysis the root cause of accident and
apply necessary corrective actions to prevent such accidents to re occur.
C) Incident Analysis –

For minor incident and no questionable conditions, report shall be based on


information provided and ensuring corrective action.

If the actual or potential severity of the accident or behaviour is significant or


occurs frequently, a special committee shall conduct detailed investigation.

The committee will investigate and review all details for root cause discovery to
determine the underlying causes and to suggest a corrective action.

All corrective action shall be carried out, monitored by Site-In-Charge, safety head.

All investigation and corrective action record shall be documented and


properly maintained.

While investigating an accident simple questions shall be asked and information


collected.

D) Record keeping-

First aid record shall be maintained in detail like how the injury occurred, what
medical care has been done.

Records of Serious / Fatal Injuries shall be properly documented and maintained.

E) Serious / Fatal Injuries –

All records relating with serious, fatal injuries shall be maintained as per the guidelines
of client.

F) Monthly Safety Performance Report –

Safety Manager shall prepare a monthly safety performance report which


contains the following:

1 Number of man hour worked on site

2 Number of total days worked

3. Number of safe days worked

4 Non-lost time injuries

5. Days lost to injuries for the orgnisation.

G) Learning’s –

1 Near miss incident report data shall be analysed for trends to identify.

2 Corrective actions shall be taken to maintain the highest level of safety and
health performance.

24 Fire Prevention and Protection
Necessary compliance shall be done for Fire Prevention and Protection on project site.

A) Necessary training shall be provided for employees for fire prevention and following
steps shall be taken.

Inspection of equipments for proper operation and condition.

Audits and inspections of workplaces and activities.

As per requirement portable fire extinguishers shall be available on site.

Also fire water bucket, sand water bucket shall be used on site.

During hot work, all required fire prevention system shall be maintained at
concerned workplace.

Proper training shallbe provided to all employees regarding-

Types of fires and use of fire extinguishers to be

used. Operating of fire extinguishers

Generally CO2 and dry chemical powder fire extinguishers shall be available at
work area.

Before start the welding or hot work, the work area shall be inspected.

sparks or molten metal won’t fall on combustible material.

Matches shall not be used to light torches. Spark lighters shall be used.

Where there is a high risk of fire or risk of injury to other workers, at such places fire-

B) Inspection of Fire Extinguishers-

All the Fire Extinguishers shall be checked or inspected at specific intervals.

Records of all extinguishers, which are placed at various locations, shall be

maintained. At site, sufficient fire extinguishers shall be maintained.

Fire extinguishers which are found defective shall be immediately serviced or replaced..

C) Reporting Emergencies-

All fire related accidents shall be reported immediately. Report will

contain the following information :

25 Environmental Hazards
All necessary precautions shall be taken on project site.

During welding, gas cutting or any heat process local exhaust or ventilation shall
be provided. If necessary, respirators shall also be used.

Oils, grease, fuels, lubricants, paints, acids or any type of chemicals or contaminated
waste water shall not be allowed to discharge on the ground or into water.

Spillage of petroleum product, chemicals or others materials shall be restricted.

Washing and maintenance of vehicles shall not be at project site, it will be only at
specific areas.

Scrap metal generated from a project site shall be properly disposed on proper basis.

Health and Hygiene Standards-

Employees and workers shall use the latrines and urinals provided at site and at
labour colony.

26 Emergency Response Procedures
A) As the mechanical construction work can be very hazardous, emergency
response procedures shall be planned.
Through this policy, we can prevent accidents, fires, explosions etc. By

using this procedure, we can minimize the loss of life and property.

Emergency action plan shall be prepared in Hindi, English or any local language and l
displayed at the work place on site.

B) Training-

All sufficient and necessary training shall be provided to all employees.

This will be provided during induction training, tool box talk and during a meeting.

Emergency procedures and important contact numbers shall be displayed at key


locations on site.

C) Emergency Evacuation Plan-

All reports of fire accidents will be reported.

For all types of emergencies, Evacuation procedures shall be planned and


implemented.

Safety head or safety officer shall give information or training to our employees.
