1.0 INTRODUCTION
The Quality Plan contained herein is divided into 23 sections and related attachments. It describes and establishes the
quality assurance and control system used by SHROMB GEOTECHNICAL LTD in performing assigned purchasing;
inspection, service supply as regard the CONSTRUCTION OF AFENO ROAD 665MX6M ASPHALT
WORKS IN IRRI COMMUNITY IN DELTA STATE –

2.0 CONTROL AND ISSUANCE OF PLAN


This plan shall be prepared, approved, maintained, and distributed by SHROMB GEOTECHNICAL LTD Quality
Assurance Engineer or his designee- QC Inspector (referred to as the "Issuing Authority"). In addition, prior to
initial release of the plan, approvals shall be obtained from SHROMB GEOTECHNICAL LTD Project Manager
and Quality Assurance Manager or their designee (Project Engineer or QA/QC Engineer respectively). Approval
signatures of these individuals shall be shown on the "third page" of this document.

Revisions to this plan shall be handled in the same manner as prescribed for initial release.

SHROMB GEOTECHNICAL LTD Quality Assurance Representatives or his designee shall issue copies of the
plan in the following manner:

2.1 Controlled Copy


Distribution of controlled copies shall be made to personnel who require continued use of the plan. The need for
issuing controlled copies shall be determined by the issuing Authority. Controlled copies shall be numerically
controlled and automatically updated as revisions or additions to the plan are issued. An authorized distribution
list for such copies will be maintained by the Issuing Authority in accordance with the Project Document
Distribution Plan.

2.2 Uncontrolled Copy


Distribution of uncontrolled copies shall be made to personnel who need access to the plan for information
purposes or limited reference and use. The need for issuing uncontrolled copies shall not be numerically
controlled nor automatically updated. Further, the issuing authority shall not be held responsible or accountable
for the status, use, or withdrawal of such copies from circulation.

2.3 Revisions

Revisions to the plan shall be identified by a revision number (e.g., Rev.00), which shall be annotated on the
cover page and in the space provided at the top of each page. When a revision is distributed, a cover letter shall
be issued by the Issuing Authority explaining the revision(s) made, and instructions for filing.

3. POLICY

SHROMB GEOTECHNICAL LTD is an integrated project management, engineering, procurement,


construction, installation, commissioning, operations and maintenance company operating in the oil and gas
industry in Nigeria, Gulf of Guinea and Western Coast of Africa.

Consistent with SHROMB GEOTECHNICAL LTD vision to become the leader in oil and gas service industry,
the epitome of excellence and quality service delivery; it is the policy of SHROMB GEOTECHNICAL LTD to
provide our clients with products and services which conform to requirements, meet clients’ needs, exceed
clients’ expectations, and are delivered in a timely and cost - effective manner.

It is recognized by SHROMB GEOTECHNICAL LTD that a total quality management/improvement programme


will favourably impact other aspects of the company activities. The company has therefore, established and
maintain a Quality Management System in accordance with the requirements of ISO 9001:2015 Standard. All
SHROMB GEOTECHNICAL LTD product and service realization and support processes are planned to meet
the requirements of this international standard and meet client’s needs in all aspects of quality, cost,
performance, safety and reliability.

The Top Management of SHROMB GEOTECHNICAL LTD has the basic responsibility over the Quality
Management System and is committed to continually improve its effectiveness.

Quality objectives in harmony with this Quality Policy are established at relevant functions, levels and processes
in SHROMB GEOTECHNICAL LTD .Adequate resources are provided to fulfill all business and quality
objectives.

The achievement of these Quality Objectives and other complementary project – specific objectives depends
largely on the quality attitude, knowledge and skill of the personnel. Therefore, highly competent and
professional workforce capable of delivering the quality products and services envisaged for clients are
engaged in the service delivery process.

4.0 OBJECTIVES

The overall aim of the project is to align the quality objectives with safety management system objectives to be
applied to this project as mentioned below:

1. To deliver the project on time and within budget.

2. Document all Project activities utilizing formal procedures in line with the requirements of ISO 9002: 1994
standard and applicable statutory regulations.

3. Use qualified and experienced personnel to perform their level of duties.

4. Ensure that services and products provided by SHROMB GEOTECHNICAL LTD are in accordance with
Contract and legislative requirements.

5. Specify the necessary Quality related to sub-contractors and vendors to ensure that required overall level
of quality and safety on the project are maintained.

6. Meet Safety, Environmental, Availability, Reliability, Operability and Maintainability targets for the project.

7. Incorporating the new facilities into the existing plant with minimal disruption whilst achieving maximum
operability.

5.0 PURPOSE

5.1 General

This PQP has been prepared as a Management summary of the Quality related activities required to meet the
terms of the contract.

The PQP sets out the management practices and Quality System, its resources and sequence of activities
pertinent to the project. It refers to SHROMB GEOTECHNICAL LTD Company members’ manuals and
procedures which (where necessary) have been modified to incorporate specific contract requirements.

5.2 Project Scope

The scope of work shall include the following:

The Project Quality Plan is based on SEPLAT requirements with due respect to QA as expressed in the
following project documents:
a) ISO 9001: 2015 Quality Management System Requirements.

b) ISO 9000: 2000 QMS-Fundamental and vocabulary.

c) ISO 9005: 2005 Management system- Guideline for quality plan.

d) Project Quality Assurance

which shall be maintained during the life of the project.

6.1 RESPONSIBILITIES
SHROMB GEOTECHNICAL LTD has in its employment responsible managers and engineers with
the responsibility to take key decisions receive progress reports and resolve issues related to the
project. The Project Manager represents the management in the daily contract management. The
job descriptions of the key project personnel, which indicate tasks, responsibilities and authorities
for project activities; reviews and approvals, are summarized below. The reporting relationships
are established in the project organizational chart.

6.1.1 Project Manager


The Project Manager as a management committee representative has the overall responsibility
and absolute authority for the management of this Project Quality Plan.

The Project Manager has the following specific responsibilities:


1. To act on behalf of SHROMB GEOTECHNICAL LTD management committee
with respect to SEPLAT and be responsible for interfacing with local authorities.

2. To review the contract documents for the project.

3. To ensure development of project procedures/instruction and a Project Quality


Plan.

4. To ensure implementation of documented procedures and practices as


appropriate to this contract.

5. To implement SPDC specific quality assurance/quality control requirements.

6. To ensure project staff are suitably qualified to perform their work

7. To initiate corrective actions.

8. To implement project auditing and make reports back to management for review.

9. The Project Manager assumes ultimate responsibility for the quality of works and
services performed under the contract. He reports to SHROMB
GEOTECHNICAL LTD Management Committee.

6.1.2 Project Engineer


1. Reports to the Project Manager on matters related to the Site activities.

2. To assist the Project Manager in his responsibilities in an alternate capacity.

3. He shall be responsible for implementation of Procedures and Method


Statements.

4. Responsible for ensuring compliance to design changes ( Change Control and
Non-Conformance )

5. He shall ensure that experienced and capable hands are involved in the
construction active.

6. He shall be responsible for the management of procurement, pre-fabrication,


assembling, transportation and installation works on site.

7. Responsible for managing HSE related to all operations on site.

8. Pointing out any deviation in cost, any delay or deficiency to the project manager
for prompt resolution.

6.1.6 QA/QC Engineer


The QA/QC Engineer reports directly to the Project Manager, with an independent
reporting line also to SHROMB GEOTECHNICAL LTD Board and his responsibilities
include:

1. Establishment, evaluation, implementation and verification of the Quality


Assurance and Quality Control systems.

2. Establishment, preparation and approval of the Project Quality Plan (PQP) and
construction Quality Plan (CQP)

3. Preparation and control of internal and external Quality Audit and Quality Finding
Report.

4. Review of quality requirements of procurement specification, vendors’ prints and


material requisition.

5. Review and approval of Inspection and Test Plan (ITP) assignment of inspection
point and performing the inspection and test for inspection point assigned.

6. Performance of source, receiving, in-process and final inspection.

7. Issue and control of Non-Conformance Report (NCR) and Corrective Action


Request (CAR).

8. Reporting quality problems and quality control status to the Project Manager.

9. Check and patrol whether the latest design documents are used at work or not,
distribution and status control of the Project Quality Plan (PQP) and Construction
Quality Procedures and Method Statements.

10. Establishment of training plan and execution of training on quality assurance


system.

11. Review of engineering procedures manual.

12. Check, monitor and evaluate the use of applicable codes, standards and
specifications in the design documents on site.

13. Review and approval of work procedures.

14. Control of Quality Assurance records, which are transferred after construction
work.

15. Quality requirement review and comment on material vendor/supplier documents.

16. Subcontractor/Vendor surveillance.

17. Maintenance and control of calibration status for measuring and test equipment.

18. Check for control activities of measuring and test equipment possessed by the
Construction Manager.

19. Supervise and ensure that calibration and control of measuring and test
equipment’s are performed in accordance with this PQP.

20. Approval for disposition plan of Non-conformance Report (NCR) and verification
for implementation.

21. Identification of conditions adverse to quality and request for corrective action to
responsible organization.

6.1.5 HSE Coordinator


The HSE Coordinator has as his main task, the responsibility of implementing HSE
programs that will achieve the reduction of exposure to hazards and accidents on the
project. In carrying out the responsibility, he shall maintain effective interface with the
Construction Manager, Project Manager and the clients. He shall be responsible for the
following:

1. Preparation and establishment of Community Affairs, Safety, Health, Environment


and Security (“HSE” hereafter) Plan.

2. Implementation of all HSE aspect of the Work particularly swimming certification


for all personnel to be engaged in the Construction phase.

3. Formulating, administering and making changes to HSE program on the project.

4. Ensuring compliance with SHROMB GEOTECHNICAL LTD & Clients’ policies


and regulations, in addition to compliance with governmental regulations.

5. Participation in corporate inspections, audits and accident investigations including


reporting and recommendations for follow-up.

6. Selection and approval of all safety equipment.

7. Monitoring and reporting of safety performance and maintenance of HSE record


systems.

8. Selection, training and supervision of all safety personnel including first aiders and
rescue crew.

9. Maintain outside professional contacts for the purpose of exchanging information


with others through which HSE activities/program could be up-dated.

10. Responsibility for organizing Base HSE meetings

11. Responsible for periodic inventory of all safety equipment

12. Enforcement of adherence by sub-contractors and vendors to HSE rules and


regulations

13. Attendance at all job-planning meetings for the purpose of determining applicable
safety procedures relevant to anticipated works.
14. Maintain liaison with the Retainer Clinic.
1. Ensuring effectiveness of various emergency response systems by activating
drills through site Safety Officers, identifying areas of weakness and taking
remedial actions.

6.1.9 Material Supervisor

6.1.10 Public Relation Officer (P.R.O)

2. Negotiates between the company and the worksite communities to maintain


cordial company / community relations.

7.0 TECHNICAL INTEGRITY

The objectives of this documented Project Quality Plan are to establish the relevant work procedures and to
guarantee the technical integrity of the installed works. Compliance with the relevant codes, rules and standards
shall be established, checked and maintained throughout the contract in accordance with the quality plan and
implementation.

8.0 CONTINUOUS PERFORMANCE MONITORING

8.1 Planning
The performance Measurement and Improvement planning on the project shall be based on a unique
precedence network indicating all identified activities at all levels. The Scheduling software to be used shall be
Microsoft Project ’98 for windows 98.

The approach to be used in the Software for Project Control shall be based on the establishment of a datum
baseline-in terms of time and resources – against which detail assessment of status can be reported in a
timely fashion.

The system offers in a flexible approach, which ensures the identification of any deviation or changes at the
earliest time, which may require an action to be taken by the Project Manager. It shall also forecast any
adverse trends in and subsequent urgent action(s) required to avert unsatisfactory situations.

Bar Charts, histograms, progress curve etc. shall be produced by aggregation through the project and across
discipline and job function for easy monitoring of the performance.

In order to effectively cover the total scope of work on the project at the required level of detail, SHROMB
GEOTECHNICAL LTD adopted the system of splitting the work scope into various categories. From this,
application of work breakdown structure shall be used “to package by unit” the entire work scope according to
Project Management demands. The gains of this to the Project Management are manifested in the inherent
flexibility, production of high quality and easy to understand graphical outputs, data integrity (source from
common data base) and fully automated output that permit dynamic update.

The progress measure in terms of actual physical work accomplished with each work item is weighted to
reflect its value in terms of the budgeted man-hour.

The exception to the above shall be the procurement performance which is weighted according to cost to
enable progress summary to be made from Level 3 to Level 1.

Construction progress shall be measured from each Construction Site to determine the actual performance,
interim quantity and quality as against the estimated base figure

8.2 Progress Monitoring and Analysis


Following the progress measurement described above, each activity shall be monitored to establish whether it
has or will be completed as planned. A weekly progress meeting involving all the team Managers and Project

Managers to review what has been accomplished during the period and confirm activities to be targeted for the
following week.

The network will be regularly updated to determine key project milestone achievement as well as check if
there is any risk to the project completion dates.

The weekly progress report should be delivered to reach the client 16hrs (real time) on Tuesday each week
indicating work progress during the previous week up to Friday at 18 hrs. The weekly progress report shall
contain the following information.

A. Overall project progress of achieved against planned

B. Reports of any scheduled activities that are in delay and the cause, plus the recovery plan and
methodology narrative of recovery.

C. A comparison of current forecast and scheduled completion dates or key deliverables.

D. Reasons for delay or anticipated delays.

E. Highlights of main events and problem areas.

F. Lost time incidents (LTI’s)

G. List of outstanding actions required from SPDC.

8.3 Progress Reporting


The Construction Manager shall be responsible for issuance of project progress report to SEPLAT for
approval. The project progress report shall be issued to SEPLAT through the Project Manager on weekly and
monthly basis. The report shall consist of two hard copies and one electronic copy.

The monthly project report this shall be delivered in five copies to the client within ten calendar days of the
ending of the preceding month. Information contained in the monthly project report shall include the following:

A. Summary narrative report

B. Community affairs, safety, health, environment

C. Security report

D. Design and Engineering

E. Procurement

F. Construction

G. Commissioning

J. Financial status

K. Nigeria content

8.4 Basis for Progress Report


1. Physical performances of all operating site as against the projected aggregate.
2. Physical progress in the area of documentation as monitored by document control unit.
3. Review and approval of applicable document by responsible Managers of Engineering, Procurement,
and Field Construction.
4. Input to the Project Planning System to produce S-Curves, forecast etc.
5. Quality Awareness/Quality Control reporting will be identified through the progress of the project
construction and scheduled audit.

8.5 Accuracy of Report


The Project Manager shall be responsible for accuracy and / or integrity of the project performance report.

Each discipline manager shall carry out a thorough review of progress report and highlight any problem areas
that need rectification before passing it to the Project Manager.

Project progress report shall not be based on assumption or projected forecast but on actual performance
against the time measurement.

9.0 REFERENCE PROCEDURE

The procedures referenced in the Quality Activity Matrices are specifically prepared in-line with the identified
activities of the contract.

Additional specific site construction/installation procedures may be prepared to guide in the performance of any
special item of work due to change request, deviation or additional work that are not covered by the existing
procedures, such as welding, piping/supports fabrication, electrical/instrumentation, mechanical static/rotating
equipment installation, civil and painting works.

The procedures shall be updated, if at any time some inadequacies are discovered in them.

A master Document Register shall be developed listing all procedures, plans, method statements and ITP’s with
their Document number, Rev No, and Date of issue.

10.0 RULES CODES AND STANDARDS

The rules, codes and standards applicable to this project shall be applied in the following order of precedence:

 Client Specifications

 National Standards

 International Standards

 Industry Standards

 Sub-contractor and Vendor working standards.

This section shall be fully prepared to reflect the input to the basis of design.

The requirements shall be listed here or be in a separately produced document referencing this section. Refer
to listing, which will be specified by the client with contract award.

11.0 CONSTRUCTION REVIEW

Project activities during the construction phase shall be reviewed in other to verify the following.

(a) Procedural compliance of all works being performed.

(b) Disposition of all non-conformance and prevention of recurrence

(c) Allocation of necessary / competent human resources and qualified personnel to the project.

(d) Information necessary to achieve the construction objectives are clearly understood and communicated
to all construction personnel.

(e) Workability and efficiency of existing construction strategy and recommend change where necessary.

The Construction Manager, Field Construction Engineer and QC Engineer, shall carry out construction review.

12.1 Materials (Electrical, Instrumentation, Painting, Consumables, Mechanical, etc.…)


All equipment, materials and consumables shall be transported, Protected, Preserved and stored according to
their manufacturers’ specifications. Material shall be handle by SHROMB GEOTECHNICAL LTD applicable
lifting procedure.

12.2 Storage
 Storage areas must be accessible to personnel that will be required in unloading and moving items
from storage to place of fabrication as the case may be.

 The storage shall have suitable fire protection, security from damage, theft, vandalism and allow
controlled disbursement of stored material and equipment.

 Hazardous chemicals, paints and solvent must be stored in well-ventilated areas and a proper
means of isolating and cleaning up spills must be in place.

 All storage shall be subjected to periodic quality checks and records maintained. Preservation &
Protection Responsibility shall rest with each sectional QC Personnel

12.3 Warehouse Storage

 The insulation materials shall be stored and controlled at indoor place.

 Bolts, nuts, gaskets and flexible hose shall be stored according to material specification and size.

 Electrical materials shall be shelved in ascending conduit size.

 Oxygen and acetylene cylinders shall be stored vertically in designated area and securely chained.

12.4 Site/Yard Storage

 Pipes, fittings, valves will be stacked on wooden timbers to keep them free from dirt, soil and
moisture. Beveled or threaded ends shall be protected to avoid damage during handling.

 Pipes in crates or in bundles shall be stocked as they are and the pipe ends protected with plastic
end covers.

 Stainless steel material shall be segregated separately from carbon steel, low allow steel or gilded
materials.

 Flange faces shall be protected by applying rust preventives as stated in and protected with
wooden cover.

13.0 TEST FACILITIES

13.1 Site Facilities Weld Quality Pipe / Structure cleanness (Surface Profile Gauge),
Paint thickness (WFT and DFT Gauges), among others.

13.2Testing and Reporting

All results obtained during testing, production, supplying, placing/pouring, finishing, curing, striking, welding, etc.
and weather conditions shall be recorded and reported.

14.0 PROJECT QUALITY AND TECHNICAL AUDITS

14.1 General Scope


Internal and External Quality and Technical audits shall be programmed and conducted on regular basis
throughout the duration of the project.

It shall be the responsibility of the Project Quality Assurance Engineer to plan and conduct quality and
technical audits to:

(a) Determine the adequacy and effectiveness of the quality systems, procedures and practices.

(b) Provide an objective evaluation of compliance with applicable code standards and procedures,
including ISO 9000 series standards.

14.2 Requirement
Auditors shall be independent from those having direct responsibility for the performance of the activities to
be audited.

Auditors shall be qualified in accordance with recognized industry standards/practices such as ISO 10011-2.

Construction quality/Technical audits shall be performed in accordance with a written procedure. The
construction audit schedule shall include both internal audit and audit checklist to be used.

The quality/technical audits shall contain such items as the identification of groups to be audited, audit scope,
audit team members, audit schedule and audit checklist to be used.

Audit schedule shall be planned to coincide with predetermined stages of the project milestone on each site.
Initial audits shall highlight deficiencies or compliance of the system being verified. These findings shall be
used to determine the frequency of future audit.

14.3 Responsibilities
The Project QA Engineer shall be responsible for providing audit checklist as well as advance notification to
SEPLAT.

The audit team leader shall be responsible for preparing audit report.

The Project QA Engineer shall select the audit team leader and auditor(s) who have an experience and
training which are commensurate with the special nature of the activities to be audited.

The Project QA Engineer shall prepare a quality and technical audit program at the beginning of a calendar
year for internal quality and technical audit to be carried out. This program shall be revised as necessary to
assure complete coverage of all activity units.

14.4 Performance Of Audit


The audit team leader and his members shall perform the audit using audit checklist. The quality/technical
auditing shall include interviews with the key personnel, review of procedures for adequacy and content review
of work and records to verify implementation of QA/QC program and general assessment of the effectiveness
of that program in-term of whether or not it accomplishes the intended purpose.

14.5 Audit Result/Report


The audit report shall include the following information as minimum and a copy submitted to SPDC.

1. Applicable sections of the project quality plan, procedure and standard in which the deficiency was found.

2. Description of each deficiency or observation

3. Name and signature of the audit team leader.

Improvement Results (IMR’s) shall be issued when a potential non-conforming situation exists or when an
improvement to the process should be made. Corrective Action Requests (CAR’s) shall be issued when there
is deviation from requirements.
The Head of audit team shall return the audit report within ten (10) days from the receipt of it the audit team
leader after filling out the corrective action column for the audit report.
The audit team shall verify and record the implementation and the effectiveness of the corrective action
through one or all of the following follow-up steps.

1. Performing direct audit in the same manner as original.

2. Evaluating all documentation, that could be attached on the returned audit report.

15.0 AUDITS AND CLIENT REVIEW

The Project Quality Assurance Engineer shall conduct periodic reviews of audit program report of concluded
audit to ensure adequacy and timely implementation of improvement request and Corrective Action.

In addition this review shall confirm whether or not the response contents and verification for corrective action
including counter plan to prevent recurrence are required.

Report of the status of the outstanding and Corrective Action Request shall be issued to the Project Manager on
monthly basis.

After verifying the above, the Project Quality Assurance Engineer shall submit the audit report to the Project
Manager for his approval.

16.0 .METHODS OF UP-DATE

This Project Quality Plan shall be reviewed in sections and/or updated as the case may be by the Project QA
Engineer when significant changes in the terms of the contract occur or activities are found to be significantly
inappropriate due to findings during construction, quality or technical audit.

The Project QA Engineer is responsible for the updating of the Project Quality Plan.

Distribution of the up-dated Plan shall be the responsibility of Project Quality Assurance engineer and the
Project Manager.

17.0 RESOURCES
The resources for this project are human and material; the human resources include all the personnel: All
personnel on board must possess swimming and other relevant Certificates, and must be qualified for the
aspect of work they are engage to perform.

18.0 NONCONFORMITIES

All processes, materials, equipment must conform to applicable standard in the industry. Any material or
equipment that does not conform shall be identified and rejected on grounds of nonconformance. Such
processes shall only be allowed after they have been corrected and or replaced. SHROMB GEOTECHNICAL
LTD QA/QC officer shall separately or jointly inspect all processes, equipment and materials with the client
representative. Report made on nonconformance must be closed out and signed by the client representative
before proceeding to the next stage.

19.0 MONITORING AND MEASUREMENT

Project progress shall be daily monitored through a strict compliance to the work program. Manpower I
equipment deployed on site shall be the index of this measurement. It is therefore required that only properly
Certified and pre-mobbed equipment with a healthy workforce can make us achieve our goal. Records of
broken down equipment and or sick persons which may adversely affect the man hours and production shall
be properly kept.

20.0 MATERIAL RECONCILIATION

At the end of the project, material reconciliation is properly undertaken with the Project Engineer/his
representatives. The essence of this is to ensure proper usage of the issued materials together with the
shortfalls procured to ensure that they are well accounted for. These include the line pipes, structural materials
fittings, the coating materials, primer etc.

All excess materials not used on the project shall be returned to SEPLAT warehouse after a proper
documentation.

21.0 DISTRIBUTION

The Project Quality Plan is a controlled document and shall be distributed by the document controller to all line
Managers and holders of procedures. The quality plan shall be issued in accordance with the document
distribution.

The controlled copies shall be stamped “CONTROLLED” to provide easy identification. Extended distribution
may be made as necessary by Assistant Project Manager to unit supervisors. The extended distribution shall be
made essentially of the photocopies or originally controlled copy. The red ink stamping of original controlled
copies serves to distinguish them from photocopies.

Original hard copies of each revision shall be retained by the Document Controller for our historical record.
Electronic files of the current revision of Project Quality Plans and procedure shall be stored in server directory.

The document controller shall be responsible for updating the project document index. Additional Controlled
Copies may be provided upon request to the Project Quality Assurance Engineer.

When it becomes necessary to make revisions vertical lines in the right hand side margin of the affected text
shall identify them. All pages, excluding attachments will be raised to the same revision status and the entire
document reissued in line with Section 2.c above, under cover of a revised Project Quality Plan table of content.
Each recipient shall be responsible for keeping his copy of the Project Quality Plan up-to-date.

22.0 DOCUMENT DISPOSAL


When work is completed, the documents will stay for six months before they are disposed
