ENROLLMENT PROCEDURES FOR NEW STUDENTS (With Entrance Exam) 1st year

Steps 1 to 3 took place from June 1 to August 27, 2021.

Reminders: Before you fill up the link on Step 1, please take note of the following:

1. Make sure to enter active Gmail account.


2. Make sure to enter active cellular phone number/s.
3. Make sure to enter your Messenger account.

Step 1. Accomplish New Student- Applicant Form for Freshmen (NSAFF) in this link
(For qualified enrollees only: Please wait for the message of your concerned institutes for the
link.).

At the end of NSAFF, a link for recommendation letter will appear

This said link must be forwarded by the student-applicant to two of the following for their letter
of recommendation: school principal, guidance counselor, class adviser, or subject teacher.

Step 2. Submit soft copies of the following documents in this link

2.1 F-137
2.2 F-138 (Report card showing at least grades in the third grading period)
2.3 Barangay certificate as a proof of residency

Step 3. Upon submission of the soft copies of documents in step 3, the schedule of entrance
examination and link will be sent through your email address.

Steps 4 to 5 took place from Sept. 10 to Sept. 29, 2021.

Step 4. Please refer to CCA Admission & Registrar's Office’s Facebook page

Steps 1, 2, and 3 will be included in the list.

Step 5. Wait for a call (either via cellphone or messenger account) from an Academic
Coordinator or an Institute Dean for an interview.

Steps 6 to 8 will take place from October 6 to 8, 2021.

Step 6. Expect a confirmation note for enrollment via text message or email from the
Academic Coordinator or an Institute Dean within five working days after the scheduled
interview. The link for enrollment will be sent to you thru email or messenger account.

Step 7. After receiving the confirmation note for enrollment, submit the following documents
ON THE DATE to be given through your email account.

7.1 Original F-137 ( IF AVAILABLE )
7.2 F-138
7.3 Photocopy of original school ID from previous school or any valid/official ID with
signature
7.4 Original PSA birth certificate
7.5 2 1.5x 1.5 identical photos with name tag and white background
7.6 Certificate of Good Moral Character

Place the documents inside a long brown envelope and write the following information at the
topmost left of the envelope.

Kindly submit the envelope at the 2nd floor of Angeles City Library and Information Center (near
Holy Rosary Parish Church).

Step 8. For enrollment, please accomplish the Enrollment Form for Freshmen (EFF) in this link
(For qualified enrollees only: Please wait for the message of your concerned institutes for the
link.).
which will be emailed to you through your email account.

Upon accomplishment of the EFF, kindly wait for five working days. The Institute concerned
will email your e-COR through your email address.

Step 9. Upon receipt of the e-COR, kindly affix your signature and scan it. Please send your
scanned e-COR to the same email address (institute email address), which initially sent it to you.
The acknowledgment of your e-COR confirms your official enrolment. Please see the attached
sample of e-COR.

Step 10. The Enrollment Confirmation Certificate (ECC) will be released within 10 working
days from the start of classes to your Class President.
Classes will start on October 18, 2021.

(students with no failing marks in the previous semester and will take the full load for the
semester) 2nd year to 4th year

The enrollment will take place from October 4 to 8, 2021.

Step 1. Accomplish the Enrollment Form for Old Regular Students Form (EFORSF) in this
link (For qualified enrollees only: Please wait for the message of your concerned institutes for
the link.).
Step 2. Upon accomplishment of EFORSF, kindly wait for five working days. The Institute
concerned will email your e-COR through your email address.

Step 3. Upon receipt of the e-COR, kindly affix your signature and scan it. Please send your
scanned e-COR to the same email address (institute email address), which initially sent it to you.
The acknowledgment of your e-COR confirms your official enrolment. Please see the attached
sample of e-COR.

Step 4. The Enrollment Confirmation Certificate (ECC) will be released within ten (10) working
days from the start of classes to your Class President.

Classes will start on October 18, 2021.

ENROLLMENT PROCEDURES FOR OLD IRREGULAR STUDENTS (students with
failing marks but qualified to enroll as approved by the Dean of the institute concerned)

The enrollment will take place from October 11, 2021 to October 15, 2021

Step 1. Accomplish the Enrollment Form for Old Irregular Students Form (EFOIrSF) in
this link.(For qualified enrollees only: Please wait for the message of your concerned institutes
for the link.).

Step 2. Wait for a call (either via cellphone or messenger account) from your academic
coordinator for enlistment of subjects.

Step 3. After the enlistment of subjects, kindly wait for five working days. The Institute
concerned will email your e-COR through your email address.

Step 4. Upon receipt of the e-COR, kindly affix your signature and scan it. Please send your
scanned e-COR to the same email address (institute email address), which initially sent it to you.
The acknowledgment of your e-COR confirms your official enrolment. Please see the attached
sample of e-COR.

(as approved by the Dean of the institute concerned)

The enrollment will take place from October 11, 2021 to October 15, 2021

Step 1. Accomplish the Enrollment Form for Returning Students Form (EFRSF) in this link
(For qualified enrollees only: Please wait for the message of your concerned institutes for the
link.).
Step 2. Wait for a call (either via cellphone or messenger account) from your academic
coordinator for enlistment of subjects.

Step 3. After the enlistment of subjects, kindly wait for five working days. The Institute
concerned will email your e-COR through your email address.
