AFFIDAVIT

I, Police Officer 3 Mark Tolosa, of legal age, single, Filipino, with residence at
Brgy Tibay, Tanay Rizal, after having been duly sworn to in accordance with law do
hereby depose and say that:

1. I am an investigating officer of the Philippine National Police stationed at

Tanay Police Sation for 12 years;

2. That on July 8, 2024 at around ____ in the afternoon, a certain Mrs.

Calbugan came to our office and reported that his son “Jayson Calbugan”
is missing;

3. I, together with my co-police officer went immediately to Sitio Tampaka,

Brgy. Tibay where the alleged suspect resides;

4. Upon searching the place, we came across to the victim, running to us

with bruises all around his face and body;

5. I, together with my companions immediately rushed him to the hospital to

check on him due to bruised found on his face and body;

6. That I am executing this affidavit in support to the Opposition to the

Petition filed by Mrs. Calbugan.
