3.0 REPORTING ELECTRICAL ACCIDENTS, INCIDENTS AND


SUPPLY FAILURES
3.1 OBJECTIVE

3.1.2 The Electrical Safety Rules require reports to be made in accordance with Approved
procedures in the event of abnormal electrical incidents, accidents or loss of supply.
3.1.3 The following are the Approved procedures to be followed in the event of such
circumstances.
3.2 OBJECTIONS

3.2.1 The Electrical Safety Rules directs that any person who objects to performing
operations or work on High Voltage or Low Voltage equipment shall make their
reasons known to the person issuing the instructions. It is expected that they will
endeavour to reach an agreement and come to a decision whether the instructions
should be amended or cancelled.
3.2.2 When an agreement cannot be reached, the person issuing the instructions will refer
the matter to a higher electrical authority for a decision or advice before issuing
further instructions.
3.2.3 The higher electrical authority will decide if a report on the objections, and the
reasons, should be submitted to the Operations Field Manager and Operations
Electrical Discipline Head.
3.3 ACCIDENTS AND INCIDENTS

3.3.1 The reporting procedure for accidents and incidents shall be as per Al Furat
Petroleum Company accident reporting procedure.
3.3.2 In addition to this procedure, for all abnormal electrical accidents or incidents, the
electrical discipline representative of the investigation team, the area Control
Person or Area Electrical Section Head shall set up and maintain a file containing
all documents related to the accident or incident. Upon conclusion of the
investigation, a summary report shall be prepared by the electrical discipline
representative, the area Control Person or Area Electrical Section Head and
submitted to the Operations Electrical Discipline Head.
3.3.3 The Electrical Safety Rules require all dangerous occurrences and electrical
accidents to be reported immediately to the area Control Person or Area Electrical
Section Head who shall report in accordance with an Approved procedure. The
Control Person or Area Electrical Section Head is not required to duplicate the
above procedure in 3.3.1 but should ensure that appropriate reports are made.

3.4 FAILURE OF SUPPLY OR ELECTRICAL FAULT

3.4.1 The Electrical Safety Rules require that any failure of supply on the High Voltage
system shall be reported to the area Control Person or Area Electrical Section
Head. Loss of supply to important Low Voltage installations and consumers shall
also be included or similarly reported.
3.4.2 The area Control Person or Area Electrical Section Head shall prepare a fault
report utilising the database system for this purpose located in the Company
electronic network. The person who prepares the fault record shall inform the
Operations Electrical Discipline Head and the Power System Superintendent.
3.4.3 The area Control Person will coordinate the preparation of any further reports
required by the Operations Electrical Discipline Head.
3.5 EMERGENCY SWITCHING

3.5.1 The Electrical Safety Rules specifies conditions and deviations from normal
procedures to carry out emergency Switching.
3.5.2 When the area Control Person or Area Electrical Section Head becomes aware
that such Switching has taken place, he shall first consider what further action needs
to be taken to safeguard personnel and equipment, or to restore power supplies to
facilities and consumers.
3.5.3 The area Control Person or Area Electrical Section Head shall decide if the
seriousness of the incident and possible subsequent loss of power supplies requires
him to immediately make a verbal report to the Area Superintendent.
3.5.4 The area Control Person or Area Electrical Section Head shall then confirm this
verbal report in writing by e-mail or facsimile message to Line Management, with a
copy e-mail or facsimile message to the Operations Electrical Discipline Head.
