PROCEDURES

12.0 LOSS OF ELECTRICAL SAFETY DOCUMENTS AND ELECTRICAL


KEYS
12.1 OBJECTIVE

12.1.1 This procedure provides guidelines that shall be followed to ensure the safety of
personnel when any of the following operational situations occur.
(a) Electrical Safety Documents issued to allow work or testing on or near
Electrical Equipment are missing or lost and work or testing is not complete.
(b) Electrical Safety Documents issued to allow work or testing on or near
Electrical Equipment are missing or lost and the equipment is urgently
required to be returned to service.
(c) The recipient of an Electrical Safety Document issued to allow work or
testing, on or near Electrical Equipment, is absent from site and cannot be
traced to sign the Hand Back of Work section of the document, or to agree to
its cancellation, when the equipment is urgently required to be returned to
service.
(d) The loss of keys to Standard Electrical Locks or padlocks, Safety Padlocks
and Lock Out Boxes.

12.2 WHEN AN ELECTRICAL SAFETY DOCUMENT IS MISSING OR LOST


AND WORK OR TESTING IS INCOMPLETE

12.2.1 Work or testing shall not proceed until the following procedures have been carried
out.
(a) All members of the Working Party are withdrawn from the work or testing.
(b) A thorough search has been made for the missing document.
(c) The recipient of the document shall sign the copy in the book, from which the
document was issued, confirming that he has cleared the document. A Senior
Authorised Electrical Person shall then cancel it.
(d) A new Electrical Safety Document shall be issued to allow completion of the
work or testing.
(e) A record of the circumstances shall be written in the substation Log Book and
the safety measures taken.

12.2.2 The responsible party shall prepare a written report of the incident to the area
Control Person or Area Electrical Section Head.

12.3 WHEN AN ELECTRICAL SAFETY DOCUMENT IS MISSING OR LOST


AND THE EQUIPMENT IS URGENTLY REQUIRED TO BE RETURNED
TO SERVICE

12.3.1 The equipment shall not be returned to service until the following procedures have
been carried out.
(a) A thorough search has been made for the missing document.

(b) All members of the Working Party have been withdrawn by the Electrical
Person in charge of the work or testing, informed that the document is to be
cancelled, and that the equipment is to be made Live.
(c) The recipient of the document shall sign the copy in the book from which the
document was issued, confirming that the work or testing is complete and that
he has cleared the document. The Senior Authorised Electrical Person shall
then cancel the Electrical Safety Document.
(d) A record of the circumstances and the safety measures taken shall be written in
the substation Log Book.

12.3.2 The Senior Authorised Electrical Person shall prepare a written report of the
incident.

12.4 WHEN THE RECIPIENT OF AN ELECTRICAL SAFETY DOCUMENT IS


ABSENT AND THE EQUIPMENT IS URGENTLY REQUIRED TO BE
RETURNED TO SERVICE

12.4.1 The equipment shall not be returned to service until the following procedures have
been carried out.
(a) All reasonably practical attempts to contact the recipient of the Electrical
Safety Document shall have been made.
(b) A Senior Authorised Electrical Person shall check that the work or testing is
complete, and that any members of the Working Party who may be available,
are informed that the Electrical Safety Document is to be withdrawn and
cancelled and the equipment is to be made Live.
(c) If the recipient is employed by Al Furat Petroleum Company, then notices
warning him that the equipment is Live and advising him to immediately
contact the Senior Authorised Electrical Person, shall be posted in his
normal work place.
(d) In addition, if the recipient is employed by a contractor working for Al Furat
Petroleum Company, then the local manager or representative of that
contractor shall be directed to inform the recipient upon his return, that his
Electrical Safety Document is cancelled and that he shall contact the Al Furat
Petroleum Company Senior Authorised Electrical Person and shall not
approach the equipment detailed on his Electrical Safety Document as it will
be Live.
(e) The equipment that is to be made Live shall have Danger Notices attached or
posted around it, warning persons that the equipment is Live. If necessary, a
person shall be posted at the work site to warn the recipient of the Electrical
Safety Document that he cannot return to work on the equipment that is now
Live.
(f) If the equipment, on which the Electrical Safety Document was issued,
includes an overhead line circuit, then Danger Notices shall also be attached to
poles or towers on the route where work was to have taken place under the
document.

(g) The Electrical Safety Document shall be cleared and cancelled by the Senior
Authorised Electrical Person.
(h) The Senior Authorised Electrical Person shall inform the area Control
Person or Area Electrical Section Head of the position and the measures he
has already taken to ensure safety of personnel and the further steps he intends
to take.
(i) If Safety Padlock keys for the Isolation and Earthing of the equipment have
been secured in a Lock Out Box, and a key of the Lock Out Box given to the
missing recipient of the Electrical Safety Document, then the Senior
Authorised Electrical Person shall if necessary obtain the Safety Padlock
keys from the Lock Out Box by force.
(j) A record of the circumstances and the safety measures taken shall be written in
the substation Log Book.

12.4.2 The Senior Authorised Electrical Person shall prepare a written report of the
incident.

12.5 LOSS OF KEYS TO STANDARD ELECTRICAL PADLOCKS OR LOCKS

12.5.1 In the event of loss of keys, the following actions shall be taken:
(a) A thorough search shall be made for the missing key.
(b) The Control Person or Area Electrical Section Head shall be informed that
the key is missing.
(c) A written report of the incident shall be made which shall specify the persons
involved and key numbers of the lost keys and shall be forwarded to the Area
Superintendent.

12.6 LOSS OF KEYS TO SAFETY PADLOCKS AND LOCK OUT BOXES

12.6.1 The equipment shall not be returned to service until the following procedures have
been carried out.
(a) A thorough search has been made for the missing key and all members of the
Working Party and the area Control Person or Area Electrical Section
Head have been informed that the key is missing.
(b) When work is complete, a person designated by the area Control Person or
Area Electrical Section Head shall visit the worksite to supervise the safe
removal of the Safety Padlock or forced opening of the Lock Out Box of
which the key is missing. All members of the Working Party shall
subsequently be advised that the Electrical Safety Document is to be
cancelled.

12.6.2 A written report of the incident shall be prepared which shall specify.
