JOB DESCRIPTION

POST: Clinical Fellow in Coronary Intervention

Pay Scale: MN37

ACCOUNTABLE TO: Clinical Lead

JOB SUMMARY
The post is full time at the Liverpool Heart & Chest Hospital. The successful applicant will work
under the supervision of the ten interventional cardiologists at the trust and also for the joint
appointees. There will be exposure to all aspects of clinical cardiac intervention. Outpatient work will
involve general cardiology and pre-assessment for day-case PCI. The appointee will be jointly
responsible for management of patients transferred to the LHCH with acute coronary syndromes
and take part in the primary PCI programme.

The successful candidate will receive intensive training in all aspects of interventional cardiac
intervention including rotablation, lithotripsy, LASER atherectomy, IVUS (VH), OCT, pressure wire,
embolic protection, thrombus aspiration. It is anticipated that suitable candidates would be
independent operators in all but the most complex cases certainly by the end of the twelve-month
post and earlier in more advanced trainees.

There will be regular emergency cover as part of the cardiology SpR rota which will be closely
supervised by consultant staff. The post-holder will gain considerable experience of the
management of cardiac patients in a tertiary setting. There is a full shift rota in place, 1:15 band 1a
inclusive of prospective cover.

There will be a commitment to contributing to the rota for ACS PCI weekend lists alongside the on-
call interventional consultant. This is separate to the on-call commitment and will provide a valuable
training opportunity.

Specific duties

Outpatient clinics- Two clinics per week


Available for pre assessment

Catheter Lab - Up to six catheter lab sessions per week with attendance at further sessions for
specific advanced training.
Ward Work - The trainee will attend one consultant ward round per week and will supervise the day
to day work of the senior house officers/trust doctors in conjunction with the other PCI trainees.

PRINCIPAL ACCOUNTABILITIES
 Catheter lab sessions – 3 days per week
 On call -Tier 2 general cardiology and PPCI on-call out of hours
 ACS weekend lists
 Ward Work - The trainee will participate in the registrar of the week on-call rota and will
supervise the day to day work of the tier 1 trainees.
 The appointees will be encouraged to cross cover sessions when possible to maximise
training opportunities.

GENERAL STATEMENTS
Liverpool Heart and Chest Hospital (LHCH) provide medical and surgical adult cardiothoracic services
to the people of Merseyside, North Wales, Isle of Man, parts of Lancashire and Cheshire.
This catchment area has a population of 2.8 million people.

The Centre provides a full range of invasive and non-invasive diagnostic cardiac and pulmonary
facilities. Thirty sessions of coronary and structural intervention are undertaken per week. During
the last year 2300 PCI procedures and over 2500 invasive coronary diagnostic procedures (including
pressure wire/IVUS/OCT) were undertaken. There are 6 TAVI lists, 2 structural (ASD/PFO closure,
paravalvular leak closure) lists, 1 LAAO closure and 3 ACHD intervention lists per month.

The Centre is a foundation trust, which has seen a great deal of development in the last few years.
As a result of major capital investment the Centre is able to provide:

 180 medical and surgical beds


 26 day case beds
 12 bed cystic fibrosis ward
 10 CCU beds; 24 surgical ITU beds (potential to expand to 30)
 8 dedicated cardiothoracic surgical operating theatres
 Endoscopy suite
 Recovery room
 X-ray department with a multi-channel spiral CT Scanner and MRI scanner
 Pulmonary function and oesophageal laboratory
 5 cardiac catheterisation laboratories
 One dedicated pacing theatre and a shared pacing/aneurysm theatre
 Research laboratories
 Electronics workshop
 Pulmonary function and oesophageal laboratory

RESEARCH/TEACHING/ADMINISTRATION
Equivalent to two half days per week. The post-holders will be expected to take a minimum of a half
day for research/private study each week. Research participation is expected and the post-holder
will be given a project to carry out with a view to publication within the time frame of the fellowship.
Attendance at national and international meetings will be encouraged, particularly for presentation
of data.

The centre undertakes teaching of all final year under graduates from the University of Liverpool
Medical School and is also recognised for the training of nurses and other professional and technical
staff. The post-holders will be involved in teaching these groups of staff and other junior doctors.

A regular programme of conferences, lectures and seminars takes place within LHCH. The centre is
committed to the principal and practice of clinical audit in which all medical staff are expected to
participate. The appointees will be expected to attend and take part in the various educational
meetings. They will be expected to present cases, develop an audit interest and present the data
locally.

A specialist library is available on site. In addition, the libraries of the Liverpool Medical Institution,
Liverpool University and Broadgreen Hospital are also available for reference. There is also a purpose
built doctors’ mess, including office space, computing facilities and recreational facilities.
RECORD OF TRAINING

All training is subject to annual assessment by an educational supervisor and the programme
director. Educational supervisor will be assigned at the start of the fellowship and will provide close
supervision to ensure training goals are obtained. As well as serving as a review and training, this will
allow planning and flexibility within the post-holders’ programmes and will allow for career advice
and development of specific interests accordingly. Although not within a formal training programme
the disciplines of the “usual” SpR commitment will be applied.
Post-holders will be expected to keep a log book of all practical procedures undertaken, which will
be regularly reviewed by their supervising consultant and signed accordingly.

ANNUAL LEAVE AND STUDY LEAVE


The post-holders will have full entitlement to annual and study leave. All leave arrangements are
subject to approval by the administrative SpR and the supervisory consultant. In addition, study
leave applications are also subject to approval by the Postgraduate Clinical Tutor.

CONFIDENTIALITY

Confidentiality and Information Security.

RISK MANAGEMENT
The Trust is committed to approaching the control of risks in a strategic and organised manner.

The post holder must be aware of their individual responsibilities as detailed in the Trusts Risk
Management, Health & Safety and Incident policies, and those under the Health and Safety at Work
Act. This includes the reporting of any untoward incident, accident, potential or actual hazard
identified.

LOCAL AREA
Liverpool is a dynamic maritime city. It has experienced a major revival over the past few years and
is now recognised as a World Heritage Site. It is unusually compact and a journey to the centre
rarely takes more than 30 minutes. Merseysiders enjoy a superb quality of life.

The county abounds with fine architecture, live cultural pursuits and outstanding sport. There are
many fine golf courses in the area and the Open Golf Tournament returned to The Royal Liverpool
Golf Club at Hoylake in 2006. There are two famous Premiership Football clubs in the city. There are
numerous sailing clubs and facilities for windsurfing. Tourism has become a major industry and
twenty million people visited the city last year, attracted by the area’s natural and man made assets.
Merseyside’s theatres, museums and art collections are now amongst its strongest assets. The city
is alive with independent theatre groups, authors, poets and dance groups. Photography, sculpture,
street art and video makers abound. The Walker Art Gallery contains a collection equal to any in the
provinces and the Tate Gallery opened in 1988 on Liverpool’s waterfront at the refurbished Albert
Dock. The Lady Lever Art Gallery at Port Sunlight also houses a collection of National importance.
The Philharmonic Orchestra is nationally acclaimed and there is a full range of musical activities of all
types.

Liverpool was European Capital City of Culture in 2008, which attracted major further investment
over the next few years.

There are both Anglican and Roman Catholic Cathedrals and facilities for worship for every other
faith in the city. There are excellent schools for both sexes, including a variety of denominational
schools, both on the private and state sector.

Very good housing in the re-developed docklands, pleasant suburbs, near the sea and in rural areas
is easily available the cost of which is rather less than the national average.

Two airports are within easy reach. John Lennon International Airport (Liverpool) and Manchester
International Airport are within half to three quarters of an hour’s driving time. There are excellent
train services and motorway links to all parts of the country with easy access to the National Parks of
North Wales, the Peak District and the Lake District, as well as to both Lancashire and North Wales
Coasts. There are direct ferry links to Belfast, Dublin and the Isle of Man.
