Parental Consent Form for the processing of Children’s Personal Information

Vodacom Bursary Application

I declare that I am the parent/lawful

guardian (“you”) of

(“the child”)

I hereby grant Vodacom consent to process my child’s personal information for the purpose of the
bursary application as detailed below. The personal information to be processed include, but is not
limited to, my child’s ID, name, surname, age, school name, grade, address, contact number, email
and emergency contact details.

I acknowledge and understand that personal information will be processed subject to the following
privacy terms:

(1) The information is processed solely for the purpose of allowing my child to apply for the
Vodacom bursary and the processing of their application.

(2) My child’s personal information shall be processed for the:

 Registration and application of the Vodacom bursary;

 Assessment and processing of the application; and

 Verification of the academic record

(3) Personal information shared with Vodacom shall be retained for as long as reasonably necessary to
process their bursary application and facilitate the process.

(4) Our processing of your child’s Personal Information will always be subject to the consent that you
provide to us in terms of this privacy consent form.
