AFFIDAVIT OF SELF ADJUDICATION WITH ABSOLUTE SALE


That I, DONY SUPAN GUZMAN, of legal age, Filipino citizen, married to Riza G.
De Guzman, and a resident of Lourdes, Tarlac City, after having been duly sworn to in
accordance with law, do hereby depose and state – THAT:
That I am the surviving legal heir of the late ANTONINA S. GUZMAN, who died
intestate on July 15, 2023 at Brgy. Aguso, Tarlac City. A copy of their Certificate of
Death is hereto attached for reference.

That the said deceased left a parcel of land, particularly described as follows, to
wit:
TRANSFER CERTIFICATE OF TITLE NO. CARP2013006040
A parcel of land ( Lot 910 of the Consolidation – Subdivision plan (LRA) Pcs-39465,
being a portion of Lot Nos. 4, 5, 6, 7, 20, 27, 31, 32, 63-83, 85-97, 99-105, 109-126, 129, 130,
132-137, 139-150, 152-154, 156 & 157, Psd—03-038889; Lot 4-C, Psd-03-038888; Lot Nos. 3-12,
Pcs-03-007375; Lot Nos. 138-A & 138-A 138-B, Psd-03-045809 and Lot 39-A, Psd-03-045875)
situated in the Barangay of LOURDES, Municipality of TARLAC CITY, Province of
TARLAC.

Bounde on the NW., along line 1-2 by FIRE BREAKS 41 on the NE., along line 2-
3 by LOT 911-A; on the SE., along line 3-4 by FIRE BREAKS 42 and on the SW.,
along line 4-1 FIRE BREAKS 41

Beginning at the point marked “1” on plan, being S. 55 deg. 44’ W., 3519.43 m.
from MBM 9, TARLAC CADASTRE, T h e n c e

N. 14 deg. 22’ E. , 44.75 m. to point 2;

S. 75 deg. 38’ E. , 146.08 m. to point 3;

N. 10 deg. 44’ W. , 44.84 m. to point 4;

N. 75 deg. 38’ W. , 148.92 m. to the point of

beginning, containing an area of SIX THOUSAND SIX HUNDRED (6600)


SQUARE METERS, more or less. All points referred to are indicated on the plan
and are marked on the ground by P.S., cyl conc. mon 15x40 cms; bearings true; date
of the original survey June 1932- May 1937 and that of the Consolidation -
Subdivision survey, executed by DEXTER C. AQUINO, Geodetic Engineer, on FEB.
11- MAR. 22, 2013 & was approved on July 3, 2013.

That the deceased died intestate without any wills or testament as to the
disposition of their estates;
That there are no known debts or obligations due against the deceased; and
That it is to the very best interest of the legal heirs hereto to adjudicate unto
themselves the above-described property in the form and manner hereinafter set forth.
NOW THEREFORE, for and in consideration of the foregoing premises and
invoking the provision of Sec. 1 Rule 74 of the Rules of Court, the said HEIRS above
named have agreed to settle the above-described properties extrajudicially and to this
effect do hereby partition and adjudicate the same unto themselves equally, subject

however to the liabilities imposed by Sec. 4, Rule 74 of the Rules of Court for a period
of two years in favor of any other possible heirs, creditors or any other person deprived
of the lawful participation over the said property of the deceased and encumbrances;
That I, DONY SUPAN GUZMAN with personal circumstances instated about by
this presents do hereby manifest that for and in consideration of the sum of THREE
HUNDRED THOUSAND PESOS (300,000) Philippine currency, which we have
received to our full and complete satisfaction from GINA S. HAPITAN, of legal age,
married to Joffrey E. Hapitan and a resident of San Vicente, Tarlac City, do hereby SELL, CEDE,
TRANSFER AND CONVEY and by these presents have SOLD, CEDED, TRANSFERRED AND
CONVEYED the above mentioned properties in favor of Gina S. Hapitan, the above described
property, free from all liens and encumbrances.

IN WITNESS WHEREOF, WE have hereunto set our hands this ____ day of
