AFFIDAVIT OF DISCREPANCY

That I, ROBERTO T. YTURRALDE, of legal age, married to ERNESTA GOMEZ, Filipino

citizen and a resident of and with postal address at Mabalacat, Pampanga, Philippines, after
having been duly sworn to in accordance with law hereby depose and state- THAT:

1. That I and my wife ERNESTA GOMEZ, are the registered owner of a parcel of land
under TCT no. (238987) 043-2023018085 located at Brgy. Santiago, Municipality of Concepcion,
Tarlac, containing an area of Ten Thousand Seventy Two (10,072) square meters;
2. That on TCT no. (238987) 043-2023018085 my name appearing therein is ROBERTO
T. ITURRALDE instead of ROBERTO T. YTURRALDE which they mistakenly entered on the
record;
3. That my true and correct name is ROBERTO T. YTURRALDE and not ROBERTO T.
ITURRALDE;
4. That I hereby request the Hon. Register of Deeds of Tarlac to correct my name from
ROBERTO T. ITURRALDE to ROBERTO T. YTURRALDE after payment of the required legal fees,
thereof;
5. That I am executing this affidavit to attest to the truth of the foregoing statements
and facts.
