analysis of the degenerative conditions of the discs and of misalignments of
the vertebral bodies are not in themselves sufficient evidence for locating a
potential subluxation. Dr. Gonstead has found that the most severely
misaligned vertebrae are often compensatory misalignments to actual
subluxations elsewhere.
Compensation
A considerable number of misalignments within the spine are compensations
instead of subluxations. A compensation1 is a vertebral misalignment that
results from a subluxation2 elsewhere, It is an effect, and every subluxation
causes one. It is similar in concept to the law, “every action has an opposite
and equal reaction.” But a compensation is usually not equal to a subluxation,
at least in its appearance on the film. They are usually much more obvious in
their degree of misalignment.
If one does not understand compensation within the spine, he will
inadvertently be adjusting the gross misalignments, i.e., the compensations.
The subluxations will remain uncorrected, and the patient will not respond.
This may be the most important reason why many chiropractors help only a
small percentage of their patients.
Fixation
Every subluxation is, at the same time, a fixation. That is, the subluxated
vertebra is fixed, or immobilized, in relation to another vertebra or
anatomical part until it is adjusted. Those misaligned vertebrae that change
position readily, as when the patient moves about, are in a compensatory
position, and do not need to be adjusted. It is the cause of the compensation
that must be discovered and corrected.
Listings
Every subluxation constitutes a structural derangement between two
anatomical parts that would be in direct apposition under normal conditions.
That is, one part has become misaligned with another.
When the misaligned part exists as a potential subluxation, as opposed
to a compensation, it is listed in the doctor’s records and on the patient’s x-
ray film. The term listing3 has come to mean the group of letters that
represents the direction of travel of a misaligned part. Thus, one’s listing of
his atlas might be ASRP, or of his fifth lumbar, PLS. Part of the listing may
also represent the point of contact on the vertebra where the thrust is
delivered when an adjustment is made. For example, the “M”, of a PLS-M
listing, indicates the mammillary process as the contact point.
The letters used in Gonstead analysis, and the words they abbreviate,
are given below:
A – anterior
P – posterior
IN – internal
EX – external
R – right
L – left
I – inferior
S – superior
T – transverse
LA – lamina
M – mammillary
SP – spinous
Film Identification
The x-ray film is always observed and analyzed as the patient would appear
to the doctor from behind. That is, when the x-ray is placed in the illuminator,
the right side of the patient is always on the doctor’s right.
A standardized procedure that would virtually eliminate the error of
marking the film on the wrong side would be to always put the x-ray marker
to the patient’s upper right-hand side. In this way, the marker would not only
show whose film it is, but also immediately orient the doctor to the right-
hand side of the patient.
When the film is being observed in the illuminator, the doctor can be
assured that the film is correctly placed if the marker is in the upper righthand
corner. Such a standardized practice would also make identification of the
film much easier when films change hands.
Film Requirements
It would be logical to begin this publication with a chapter on the procedure
for taking x-rays. However, because Dr. Gonstead is in the process of
developing new and advanced x-ray equipment, especially adapted to his
work, and to be used exclusively by chiropractors, this area of study will be
presented later.
Just a few basic points will be given at this time, relative to patient
positioning for the A-P view. This is to enable the doctor to begin taking
films of the pelvis that he can analyze using the Gonstead methodology.
1. The patient is positioned in the center of, and in front of, the bucky
table.
2. The patient then lakes small steps backwards until the first part of him
touches the bucky table. He should be standing comfortably, and should
not be pressing hard into the bucky table. Doing so decreases the value
of the film for accurate pelvic analysis.
3. The patient’s heels should be on a line parallel to the film.
4. The heels are spread apart about four inches.
It should be mentioned that accurate and proper positioning is important
in order to obtain an authentic x-ray of the patient’s spine. This does not
mean that some slight variation in position will change the patient’s spinal
architecture and nullify the film; because, as was mentioned previously, a
subluxation implies a fixation,4 and the relationship between the fixed parts
does not change as the patient moves about. It merely means that the greatest
amount of information can be taken from the film when the suggested
procedures are followed.
Standard Views Taken
The Gonstead system of analysis always includes two 14 x 36 full-spine
films, of the A-P and lateral views. There may be times when smaller than 14
x 36 films are indicated, such as a “spot-shot” of a specific area. However, in
order to insure continuity of the entire spine, the 14 x 36 film is required.
When only portions of the spine are shown in this text, it is for space-
saving reasons only. It should not be construed that smaller than 14 x 36
films are sufficient for this work.
*We use the qualifying term potential because in chiropractic terminology the word subluxation, when
used alone, always implies nerve involvement. Whether nerve interference exists at any given time must
be determined by means other than through the analysis of the disc.

The AS Ilium
As an ilium misaligns anteriorward on the sacrum, it also goes in a superior
direction. This is due to the nature of the auricular surfaces of the joint, and
because of the angle of the pelvis itself. The ilium travels in an arc, in an
anterior and superior direction, away from its normal relationship with the
sacrum. The designation of such a misalignment is an AS Ilium (Fig. 1).

meillä ja kysyi: "No, Liina, tuletko tänäpäivänä?"
kuullutkaan. Jansu huomasi tämän, eikä pyytänytkään minua enää.
Tämän kuultuaan, nousi Otto äkkiä ylös, otti minua kädestä kiinni
ja kysyi: "kuka siellä on? Liina ei saa meidän luota mennä!"
"No, no, nuori herra", vastasi Leena, "elkää te tuota talonpoikaista
tyttöä estelkö, te iso saksa, antakaa hänen mennä omaa
säätyläistään tervehtimään." Ja tämän lausui hän semmoisella
äänellä ja katseella, että minä rupesin pelkäämään. Silloin astui
pieni petsattu kirjakaappi. Sinne, omaan kamariinsa, vei nyt Jansu
minut. Minä olin siellä ennen kymmeniä kertoja leikkinyt.
"Jansu", kysyin minä heti, kun sinne tulimme, "olisiko sinun isälläsi
aikaa puhumaan meille omia kertomuksiaan?"
