ANNEX ‘C’

PAKYAW CONTRACT AGREEMENT

This AGREEMENT, made and entered into this 5th day of December , 2023 by and
between the Barangay Agreement represented by SERVANDO D. AMORA , hereinafter
referred to as the BLGU CHIEF OPERATING OFFICER and JULITO M. LICONG
hereinafter referred to as the CONTRACTOR.

That, for and in consideration of this cost as mentioned in this contract, as negotiated and
accepted hereto attached and made an integral part of the PARTIES hereto covenant and
agree to as follows:

1. That the CONTRACTOR/S at own expense shall fully and faithfully perform all
the works for CHRISTMAS TREE MAKING FEE.

With total estimated contract amount of Php 8,000.00 pesos to be completed in

accordance with all the requirements of the corresponding PLANS AND
SPECIFICATIONS and in full compliance with all the terms and conditions and
instructions contained in the GENERAL CONDITIONS,APPLICATION FORM
FOR PAKYAW CONTRACT, other essential documents, which are hereby made
and acknowledge as an integral part of this agreement.

2. That the total estimated contract price in the amount of Php 8,000.00 pesos the
CONTRACTOR agrees to complete the work in accordance with the plans and
specifications:

IN WITNESS WHEREOF THE PARTIES aforesaid have hereto affixed their signature
on the day and year first above written.
